THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that item 12a (Date Entered AD [Active Duty] This Period) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 5 November 2006 be corrected to show 
24 November 2004. 

2.  The applicant states, in effect, that it was typed incorrectly. 

3.  The applicant provides DD Forms 214 for the periods ending 5 November 2006 and 30 May 2004, in support of his application.

CONSIDERATION OF EVIDENCE:

1.  Having prior service in the U.S. Marine Corps, the applicant enlisted in the Regular Army on 24 November 2004.  On 5 November 2006, he was honorably discharged by reason of physical disability, existed prior to service PEB [physical evaluation board].

2.  Item 12a on the applicant’s DD Form 214 for the period ending 5 November 2006 shows the entry, “2000 11 24.”

3.  On 6 October 2005, the Army Board for Correction of Military Records (ABCMR) granted relief of the applicant’s prior application by correcting his enlistment contract to show he enlisted in the Regular Army on 24 November 2004.  On 26 October 2005, his enlistment document was amended to show he enlisted in the Regular Army on 24 November 2004.
DISCUSSION AND CONCLUSIONS:

Evidence of record shows the applicant enlisted in the Regular Army on 
24 November 2004.  Therefore, item 12a on his DD Form 214 for the period ending 5 November 2006 should be corrected to show this date.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: 

	a.  deleting the entry, “2000” in item 12a on his DD Form 214 for the period ending 5 November 2006; and 

	b.  adding the entry, “2004” in item 12a on this DD Form 214.
