THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that her deployment to Kuwait be annotated on her separation document (DD Form 214).
2. The applicant states, in effect, that Item 18 (Remarks) of her DD Form 214 does not reflect her deployment to Kuwait in support of Operation Iraqi Freedom (OIF).
3. The applicant provides Headquarters, Fort McPherson, Fort McPherson, Georgia, Orders 336-001, dated 1 December 2004 in support of her application.
CONSIDERATION OF EVIDENCE:
1. The applicantÂ’s record shows she was appointed a second lieutenant (2LT) and entered active duty on 30 May 1987. It also shows she served in military occupational specialty (MOS) 67A (Health Services), and the highest rank she attained while serving on active duty was major (MAJ).
2. The applicant's Official Personnel Military File (OMPF) contains an evaluation report that shows she was serving in Kuwait, in support of OIF, during the rating period of 10 April 2004 through 9 April 2005.
3. Headquarters, Fort McPherson Orders 336-001, dated 1 December 2004, directed the applicant's to Kuwait, in support of OIF, on or about 3 January 2005.
4. On 28 February 2007, the applicant was honorably released from active duty for the purpose of retirement, in the rank of MAJ. The DD Form 214 issued to her at the time shows she completed a total 20 years and 10 days of active military service. Item 12f (Foreign Service) shows she completed 1 year,
7 months, and 1 day of overseas service during the period covered by the separation document. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) shows she earned the following awards during her tenure on active duty: Meritorious Service Medal (4th Award); Army Commendation Medal; Army Achievement Medal (3rd Award); Army Superior Unit Award; National Defense Service Medal (2nd Award); Southwest Asia Service Medal with Bronze Service Star (3rd Award); Global War on Terrorism Service Medal; Korea Defense Service Medal; Army Service Ribbon; Armed Forces Reserve Medal; Overseas Service Ribbon; Kuwait-Liberation Medal (Saudi Arabia), and Kuwait Liberation Medal (Kuwait).
5. In connection with the processing of this case, a member of the Board staff contacted an official from the Defense Finance and Accounting Service (DFAS), who confirmed that DFAS records confirm the applicant received hostile file/imminent danger pay for Kuwait from 1 January through 30 April 2005.
6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that the source documents for entering information on the DD Form 214 will be the Personnel Qualification Record (PQR), Enlisted/Officer Record Brief (ERB/ORB), or any other document authorized for filing in the OMPF.
7. Paragraph 2-4 of the separation documents regulation contains item-by-item instructions for completing the DD Form 214. The instructions for completion of Item 18 (Remarks) state, in pertinent part, that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the following entry will be made: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that she served overseas in Kuwait was carefully considered and found to have merit. The evidence of record confirms she served in Kuwait from 1 January through 30 April 2005, as corroborated by DFAS pay records, which confirm she received imminent danger pay for this period.
2. By regulation, the deployed service of an active duty Soldier should be documented with an entry in Item 18 of the DD Form 214. In this case, the entry "SERVICE IN KUWAIT FROM 20050101-20050430" should be added to Item 18 of the applicant's DD Form 214 to document her service in Kuwait in support of OIF.

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending Item 18 of her 28 February 2007 DD Form 214 by adding the entry
"SERVICE IN KUWAIT FROM 20050101-20050430"; and by providing her a correction to her separation document that includes this change.
