THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty), with an effective date of 4 October 2005, be corrected to show his service in Afghanistan.

2.  The applicant states, in effect, that this information was not included in his DD Form 214. 

3.  The applicant provides a copy of Statement of Wartime Service from his unit adjutant; a copy of his Soldier Deployment History Outprocessing Report; and a copy of his DA Form 638 (Recommendation for Award).

CONSIDERATION OF EVIDENCE:

1.  The applicant served in the Regular Army from 2 July 2002 through 4 October 2005.  He was administratively discharged under the provisions of Army Regulation 635-200, for a pattern of misconduct with a general discharge (GD), under honorable conditions.  He was credited with 3 years, 3 months, and 3 days of active military service. 

2.  The applicant provided a copy of a Statement of Wartime Service, dated
2 March 2004, from his unit adjutant.  This statement indicates that the applicant served in Afghanistan from 17 January – 15 May 2004.  This service is not reflected on the applicant's DD Form 214.


3.  Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents (DD Form 214/5).  Item 18 (Remarks) lists the name of the country to which deployed and the inclusive dates of service in the country.  Each entry is verified by the individual's service record.  

DISCUSSION AND CONCLUSIONS:

1.  The applicant's DD Form 214 does not show his service in Afghanistan.

2.  The applicant provided sufficient evidence to show he served in Afghanistan from 17 January – 15 May 2004.  Therefore, his record should be corrected as directed below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the statement "SERVICE IN AFGHANSITAN FROM 20040117- 20040515" in Item 18 (Remarks) of the applicant's DD Form 214, with an effective date of 4 October 2005.
