THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that the Social Security Account Number (SSAN) listed on his Purple Heart (PH) Orders be corrected.   

2.  The applicant states, in effect, that the orders he received for the PH contain an incorrect SSAN, and he requests the orders be reissued with the correct SSAN.  

3.  The applicant provides the PH orders in question and his separation document (DD Form 214) in support of his application.  

CONSIDERATION OF EVIDENCE:

1.  The applicant's record shows that he enlisted in the Regular Army and entered active duty on 28 April 2004.  The enlistment record (DD Form 4) on file in his Official Military Personnel File (OMPF) lists his SSAN with the number 5 as the seventh digit.  His record shows he held and served in military occupational specialty (MOS) 11B (Infantryman), and that he served in Iraq from 30 November 2005 through 12 November 2006.  

2.  On 4 June 2006, Headquarters, Multinational Corps Iraq, Baghdad, Iraq, issued Permanent Orders 155-065, which awarded the applicant the PH for being wounded in action in Iraq on 21 May 2006.  The SSAN listed for the applicant in the standard name line of these orders listed the number 8 as the seventh digit.  
3.  On 17 August 2007, the applicant was honorably REFRAD after completing a total of 3 years, 3 months, and 20 days of active military service.  Item 3 (Social Security Number) of the DD Form 214 he was issued at the time lists the number 5 as the seventh digit of the SSAN entered.  It also shows that during his active duty tenure, he earned the PH, Army Commendation Medal; Army Good Conduct Medal; National Defense Service Medal; Global War on Terrorism Service Medal; Iraq Campaign Medal; Army Service Ribbon; Overseas Service Ribbon; and Combat Infantryman Badge.  

DISCUSSION AND CONCLUSIONS:

The record confirms the seventh digit of the applicantÂ’s SSAN is 5, as evidenced by the DD Form 4 and DD Form 214 on file in his OMPF.  Therefore, it is clear the SSAN listed in the standard name line of his PH orders, which lists the number 5 as the seventh digit, is in error.  As a result, it would be appropriate to correct these orders to reflect his correct SSAN at this time.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by issuing him a correction to the standard name line of his Purple Heart orders that lists the correct SSAN, as listed in Item 3 of his 17 August 2007 DD Form 214; and by providing him a copy of the correction to these orders.
