THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that a Department of the Army (DA) Certificate for award of the Army Commendation Medal (ARCOM) for the period 1 September 2000 through 1 August 2004 be removed from his Official Military Personnel File (OMPF) and replaced with a DA Certificate for award of the Meritorious Service Medal (MSM) for the same period.
2. The applicant states he was recommended for an MSM, but it was downgraded to an ARCOM. The downgrade was reconsidered and the MSM was eventually awarded. The ARCOM is on his records.
3. The applicant provides:
a. A self-authored memorandum dated 30 January 2008.
b. A 24 August 2004 memorandum wherein the applicant's brigade commander rescinded his request for downgrading of the MSM.
c. Department of the Army (DA) Certificate for the MSM; Permanent Order 209-12, dated 28 July 2004, Commander, V Corps.
d. Department of the Army (DA) Certificate for the ARCOM; Permanent Order 209-12, dated 28 July 2004, Commander, V Corps.
e. DA Form 638 (Recommendation for Award).
CONSIDERATION OF EVIDENCE:
1. The applicant is a Regular Army Chief Warrant Officer Three (CW3). When he departed Germany in 2004, his immediate commander recommended him for award of the MSM for his service from 2001 to 2004.
2. The DA Form 638 was originally downgraded by the Brigade Commander and forwarded to the Corps Commander who then approved an ARCOM. Later, the Brigade Commander reconsidered his decision and requested the original award of the MSM be approved. The Corps Commander then approved award of the MSM.
3. The applicant's iPERMS record contains the Department of the Army (DA) Certificate for the ARCOM; Permanent Order 209-12, dated 28 July 2004, Commander, V Corps. It does not contain the MSM Certificate.
DISCUSSION AND CONCLUSIONS:
1. The applicant was issued two awards for the same period of service in Germany from 2001 to 2004. His iPERMS record contains an award of the ARCOM for this service; it should contain an award of the MSM.
2. The applicant's records should be corrected as requested.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. Deleting the Department of the Army (DA) Certificate for the ARCOM; Permanent Order 209-12, dated 28 July 2004, Commander, V Corps, from his iPERMS record; and
b. Adding the Department of the Army (DA) Certificate for the MSM; Permanent Order 209-12, dated 28 July 2004, Commander, V Corps.

The applicant’s OMPF currently shows that she was recommended for award of a MSM and that the recommendation was downgraded by the approval authority and approved as an award of the ARCOM, 1OLC. Evidence of record shows that the approval authority at Fort Leonard Wood revoked the award due to cancellation of the applicant’s PCS. Evidence of record also shows that the ARCOM, 1OLC was never presented to the applicant, thus negating the reason for filing the DA Form 638 in her OMPF.

The DA Form 638 submitted by the applicant shows that she was recommended for award of the Meritorious Service Medal (MSM). Block 26 (Approval Authority) of the DA Form 638 shows the signature block of a brigadier general as the approval authority. Without a complete DA Form 638 awarding the applicant the ARCOM, there is insufficient evidence in which to grant her request.
