THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests reconsideration of his previous request to show service in Haiti on his DD Form 214 (Certificate of Release or Discharge From Active Duty). He also makes a new request for award of the Armed Forces Service Medal for his service in Haiti.
2. The applicant states he is providing proof of his service in Haiti.
3. The applicant provides the following new information which was not previously considered by the Board:
a. A copy of a DA Form 638-1 (Recommendation for Award of Army Achievement Medal, Army Commendation Medal, and Meritorious Service Medal).
b. A copy of an Army Achievement Medal Certificate showing Permanent Order Number 35-11, Commander, 1st Battalion, 2nd Armored Cavalry Regiment, US Army, Haiti, dated 20 September 1995, for service in Haiti from 26 May 1995 to 29 October 1995.
CONSIDERATION OF EVIDENCE:
1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20070012336 on 10 January 2008.
2. The applicant served in the Regular Army from 15 August 1994 through 16 September 1997. His DD Form 214 for this period of service does not show any foreign service and does not indicate that he served in Haiti. It also does not show his Army Achievement Medal awarded on 20 September 1995.
3. The applicant received the Army Achievement Medal for meritorious service while serving as a rifleman with the United Nations Mission in the Republic of Haiti (UNMIH) from 26 May 1995 to 29 October 1995.
4. The Awards Branch, US Army Human Resources Command (AHRC), Alexandria, VA lists the Armed Forces Service Medal as an authorized award for Soldiers who participated in the UNMIH.
5. Army Regulation 635-5, (Military Awards) prescribes the separation documents that must be prepared for soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. It states, in pertinent part, for an active duty soldier deployed with his or her unit during their continuous period of active service, the following entry will be made in Block 18, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." It also provides that the total amount of foreign service completed during the period covered by the DD Form 214 will be entered in block 12f.
DISCUSSION AND CONCLUSIONS:
1. The applicant served in Haiti as part of the UNMIH. His DD Form 214 should be corrected, in Block 12f, to show "0000 05 04"; and in Block 18, to show "SERVICE IN HAITI FROM 19950526 19951029."
2. The Armed Forces Service Medal has been awarded to Soldiers who participated in the UNMIH. This award should be presented to the applicant and placed on his DD Form 214.
3. The applicant's Army Achievement Medal should also be added to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant amendment of the ABCMRs decision in Docket Number AR20070012336, dated 10 January 2008. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. Awarding him the Armed Forces Service Medal for his participation in the United Nations Mission in Haiti from 26 May 1995 to 29 October 1995; and
b. Correcting his DD Form 214 in Block 12f to show "0000 05 04"; in Block 13 (Awards) to show the Army Achievement Medal and the Armed Forces Service Medal; and in Block 18 to show "SERVICE IN HAITI FROM 19950526 19951029."
