THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 12c (Net Active Service This Period) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 13 October 2005 be corrected to show that he completed 1 year, 
4 months, and 3 days. 

2.  The applicant states, in effect, that the entry in item 12c on his DD Form 214 was a human error, miscalculation, or a math error. 

3.  The applicant provides a copy of his DD Form 214 for the period ending 
13 October 2005.

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty on 11 June 2004 in support of Operation Iraqi Freedom.  He was released from active duty on 13 October 2005 after serving 1 year, 4 months, and 3 days of creditable active service.

2.  Item 12c on the applicant’s DD Form 214 for the period ending 13 October 2005 shows the entry, “0000 04 03.”

DISCUSSION AND CONCLUSIONS:

Evidence of record shows the applicant served 1 year, 4 months, and 3 days of active duty during the period 11 June 2004 through 13 October 2005.  Therefore, item 12c on his DD Form 214 for the period ending 13 October 2005 should be corrected.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: 

	a.  deleting the entry in item 12c on his DD Form 214 for the period ending 13 October 2005; and

	b.  adding the entry, “0001 04 03” in item 12c on this DD Form 214.
