THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 7a (Place of Entry into Active Duty) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show he entered into active duty from Portland, Maine.

2.  The applicant states, in effect, that he entered into active duty from Portland, Maine, and not from Portland, Oregon.

3.  The applicant provides his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s enlistment contract shows he enlisted in the U. S. Army Reserve (Delayed Entry Program) on 26 July 2002 from the Portland, Maine, Military Entrance Processing Station (MEPS).  His enlistment contract shows he enlisted in the Regular Army on 22 October 2002 for 3 years from the Portland, Maine, MEPS.

2.  The applicant was honorably released from active duty on 30 April 2006 after completing his required service.  Item 7a of his DD Form 214 shows he entered active duty from Portland, Oregon.  

DISCUSSION AND CONCLUSIONS:

Item 7a of the applicant’s DD Form 214 contains an administrative error.  His enlistment contract shows he entered active duty from Portland, Maine, not from Portland, Oregon.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by correcting item 7a of his DD Form 214 to show he entered active duty from Portland, Maine.
