THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 27 June 2006 to show award of the Meritorious Service Medal with First Oak Leaf Cluster.
2. The applicant states, in effect, that he had cleared and signed his retirement DD Form 214 and then received the award in the mail from his unit after he was retired.
3. The applicant provides orders and an award certificate for the Meritorious Service Medal with First Oak Leaf Cluster; and a copy of his DD Form 214.
CONSIDERATION OF EVIDENCE:
1. Having prior inactive and active service in the Army National Guard, the applicant enlisted in the Regular Army on 10 October 1986. He remained on active duty through continuous reenlistments until he was permanently retired on 27 June 2006 in the rank of Chief Warrant Officer Three by reason of physical disability.
2. The applicantÂ’s DD Form 214 for the period ending 27 June 2006 shows the Senior Aviation Badge, the Army Commendation Medal (2nd Award), the Army Achievement Medal (2nd Award), the National Defense Service Medal, the Global War on Terrorism Service Medal, the Humanitarian Service Medal, and the Overseas Service Ribbon as authorized awards.
3. Headquarters, U.S. Army Alaska Permanent Order Number 172-5, dated
14 July 2006 (the award certificate is dated 21 June 2006), shows the applicant received the Meritorious Service Medal with First Oak Leaf Cluster for the period 1 June 1996 to 31 May 2006. These orders also show the applicant received a previous award of the Meritorious Service Medal.
DISCUSSION AND CONCLUSIONS:
The orders for the Meritorious Service Medal with First Oak Leaf Cluster are accepted as sufficient evidence on which to amend the applicantÂ’s DD Form 214 for the period ending 27 June 2006 to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Meritorious Service Medal with First Oak Leaf Cluster to his DD Form 214 for the period ending 27 June 2006.
