THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that an Army Commendation Medal (ARCOM) and Nondestructive Inspection Apprentice Course be added to his
22 September 2007 Separation Document (DD Form 214).
2. The applicant states, in effect, that the ARCOM and training course indicated should be added to his DD Form 214.
3. The applicant provides a United States Air Force (USAF) Nondestructive Inspection Apprentice Course Certificate of Training and ARCOM Certificate in support of his application.
CONSIDERATION OF EVIDENCE:
1. The applicant's military record was not provided for the Board's review. However, the DD Form 214 provided by the applicant allows for a fair and accurate review of the issues in this case.
2. The applicant's DD Form 214 shows that while serving as a member of the Army National Guard (ARNG), the applicant was ordered to active duty in support of Operation Iraqi Freedom. It further shows that during this period of active duty, he served for 1 year, 4 months, and 17 days until 22 September 2007, at which time he was honorably released from active duty (REFRAD) and returned to his ARNG unit.
3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's 22 September 2007 DD Form 214 shows he earned the following awards: Army Good Conduct Medal (AGCM); National Defense Service Medal (NDSM); Global War on Terrorism Service Medal; Iraq Campaign Medal; Army Service Ribbon; Overseas Service Ribbon; Armed Forces Reserve Medal with "M" (Mobilization) Device; and Basic Aviation Badge.
4. Item 14 (Military Education) of the applicant's 22 September 2007 DD Form 214 contains the entry "None." The applicant authenticated the DD Form 214 with his signature in Item 21 (Signature of Member Being Separated) on the date of his REFRAD.
5. The applicant provides an ARCOM Certificate that shows he was awarded the ARCOM for his meritorious service in Iraq during the period 29 September 2006 through 4 September 2007, and that this award was authorized and announced in 36th Combat Aviation Brigade Permanent Orders 153-006.
6. The applicant also provides a USAF Certificate of Training that shows he successfully completed the Nondestructive Inspection Apprentice Course on
27 September 2005.
7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214.
8. Chapter 2 of the separation documents regulation contains item-by-item instructions of the DD Form 214. The instructions for Item 13 state, in pertinent part, that awards and decorations for all periods of service, as verified by the member's service records, will be entered. The instructions for Item 14 state to list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214.
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that the ARCOM and USAF Nondestructive Inspection Apprentice Course should be added to his 22 September 2007
DD Form 214 was carefully considered and found to have partial merit.
2. The evidence of record confirms the applicant was awarded the ARCOM for his meritorious service in Iraq from 29 September 2006 through 4 September 2007. As a result, it would be appropriate to add this award to the list of awards contained in Item 13 of the DD Form 214 in question.
3. The applicant's contention that the USAF Nondestructive Inspection Apprentice Course that he completed on 27 September 2005 should be added to Item 14 of his DD Form 214 was also carefully considered. However, by regulation, only formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 will be entered in Item 14 of the DD Form 214. Therefore, given the applicant completed the course in question nearly 8 months before he entered active duty on the period covered by the DD Form 214 in question, there is an insufficient evidentiary basis to support granting this portion of the requested relief.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Army Commendation Medal to the list of awards contained in
Item 13 of his 22 September 2007 DD Form 214.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to adding the Nondestructive Inspection Apprentice Course to Item 14 of the DD Form 214 in question.
