      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his Legion of Merit be added to his DD Form
   214 (Certificate of Release or Discharge from Active Duty) for the
period ending 30 April 2007.

2.  The applicant states he received the Legion of Merit after he received
his DD Form 214 and went on transition leave.

3.  The applicant provides a DA Form 638 (Recommendation for Award);
Headquarters, U. S. Forces Command Permanent Orders 54-2, dated
   23 February 2007; his retirement orders; his DD Form 214 for the period
ending 30 April 2007; and his Officer Record Brief.

CONSIDERATION OF EVIDENCE:

1.  The applicant served in Operations Enduring Freedom and Iraqi Freedom.

2.  After having had prior service, the applicant entered active duty on
           17 February 1995 as a physician assistant.

3.  The applicant was awarded the Legion of Merit on Headquarters, U. S.
Forces Command Permanent Orders 54-2, dated 23 February 2007.

4.  The applicant was released from active duty on 30 April 2007 for the
purpose of retirement.  The Legion of Merit is not listed on his DD Form
214 for the period ending 30 April 2007.

DISCUSSION AND CONCLUSIONS:

The applicant was awarded the Legion of Merit in February 2007; however, it
is not listed on his DD Form 214 for the period ending 30 April 2007.  This
DD Form 214 should be amended to add the Legion of Merit.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that the DD
Form 214 for the period ending 30 April 2007 of the individual concerned be
amended to add the Legion of Merit.
