	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that item 4a (Grade, Rate or Rank), of his DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 6 June 2006, be corrected to show the entry "SSG" instead of the entry "SGT": item 4b (Pay Grade), to show the entry "E-6" instead of the entry "E-5": item 19a (Mailing Address After Separation) and the address in 9b (Nearest Relative [Address]) be corrected to show the street name correctly identified as "Shooting Star" instead of the entry "Shotting Star": and that his records be corrected to show that he was awarded the Afghanistan Campaign Medal.

2.  The applicant states, in effect, that certain items of his DD Form 214 are incorrect.

3.  The applicant provides a copy of his DD Form 214, dated 22 October 2003 and 6 June 2006; a copy of an award certificate for the Bronze Star Medal; a copy of orders ordering him to active duty; a copy of his promotion orders to staff sergeant (SSG/E-6); and a copy of a memorandum for record, dated 29 March 2003, in support of his request. 

CONSIDERATION OF EVIDENCE:

1.  The applicant's military records show he enlisted in the U. S. Army Reserve (USAR) on 11 August 1996, in pay grade E-5, with prior Regular Army service.  

2.  The applicant was ordered to active duty effective 9 January 2003, in support of Operation Enduring Freedom (OEF), for 365 days not to exceed 2 years.  He was released from active duty on 22 October 2003 and was transferred to a troop program unit (TPU). 

3.  Item 18 (Remarks), of his DD Form 214, dated 22 October 2003, shows the entry "Service in Afghanistan from 11 February 2003 to 9 September 2003."  Item 19a (Mailing Address after Separation), of the same DD Form 214, shows his street name address as "Shooting Star."

4.  The applicant was promoted to SSG/E-6 with an effective date and date of rank of 1 April 2005.

5.  The applicant provided a copy of his award certificate which shows that he was awarded the Bronze Star Medal, on 8 September 2003, for exceptionally meritorious service as Assistant Tactical Psychological Operations Team Chief, 
Combined/Joint Special Operations Task Force, while deployed to Afghanistan, in direct support of Operation Enduring Freedom, from 14 February to 15 September 2003.  

6.  The applicant was ordered to active duty on 25 March 2005 in support of Operation Iraqi Freedom and was released from active duty on 6 June 2006.  
He was transferred to a TPU.   

7.  Item 4a, of his DD Form 214, dated 6 June 2006, shows the entry "SGT"; item 4b shows the entry "E-5"; item 12h (Effective Date of Pay Grade) shows the entry "1995 04 18"; and item 19a and 19b shows his nearest relative's street name as, "Shotting Star," which is the same street name in item 19a (Mailing Address after Separation), of the DD Form 214, he was issued on 22 October 2003. 

8.  A review of the United States Postal Service Website, shows a listing for "Shooting Star Place" and not "Shotting Star Place" in the applicant's city and state. 

9.  The applicant was released from his TPU and was honorably discharged from the USAR effective 15 December 2006, in the rank of SSG. 

10.  Army Regulation 635-5 provides instructions for the preparation of the         DD Form 214.  It states, in pertinent part, that item 4a and 4b will be completed to show the active duty grade or rank and pay grade at the time of separation.  It also states that item 12h (Effective Date of Pay Grade) will be completed to show the effective date of promotion to the current pay grade from the most recent promotion order or reduction instrument.  It also states that item 19b (Nearest Relative) will be completed to show the name and address of the nearest relative furnished by the individual at the time of separation.

11.  Military Awards Branch Message [dated 8 April 2005] provided the following implementing instructions for award of the Afghanistan Campaign Medal.  Award of the Afghanistan Campaign Medal is authorized for service in direct support of Operation Enduring Freedom on or after 24 October 2001 to a future date to be determined by the Secretary of Defense or the cessation of the operation.  The area of eligibility includes all land areas and air spaces of the country of Afghanistan.  To qualify for award of this campaign medal, service members must have been assigned or attached to or mobilized with units operating in these areas of eligibility for 30 consecutive days or for 60 non-consecutive days or meet one of the following criteria:  a) be engaged in combat during an armed engagement, regardless of the time in the area of eligibility; b) is medically 

evacuated from the area of eligibility for wounds or injuries resulting from 
participation in an operation or official duties; or c)  participates as a regularly assigned air crewmember flying sorties into, out of, within or over the area of eligibility in direct support of the military operations [each day of operations counts as one day of eligibility].

12.  Army Regulation 600-8-22 (Military Awards) states that the Global War on Terrorism Service Medal (GWOTSM) is authorized for award to members of the Armed Forces of the United States who have participated in the Global War on Terrorism (GWOT) operations outside of the designated areas of eligibility (AOE), on or after 11 September 2001 to a future date to be determined. 
Battalion commanders and commanders of separate units have the authority to award the GWOTSM for approved operations to units and personnel within his or her command. 
Service members must be assigned, attached, or mobilized to a unit supporting designated operations for 30 consecutive days or for 60 nonconsecutive days, or meet one of the following criteria:  
	(1) Initial award of the GWOTSM is limited to airport security operations (from 27 September 2001 through 31 May 2002) and Soldiers who supported Operations NOBLE EAGLE, ENDURING FREEDOM, and IRAQI FREEDOM, or
	(2)  all Soldiers on active duty, including Reserve Component Soldiers mobilized, or National Guard Soldiers activated on or after 11 September 2001 to a date to be determined having served 30 consecutive days or 60 nonconsecutive days are authorized the GWOTSM. 
Service members may be awarded both the GWOTEM and the GWOTSM if they meet the criteria for both awards; however, the qualifying period used to establish eligibility for one award cannot be used to justify eligibility for the other award.  Only one award of the GWOTSM may be authorized to any individual; therefore, second and subsequent awards will not be awarded.

13.  Soldiers may receive both the Global War on Terrorism Service Medal and the Global War on Terrorism Expeditionary Medal if they meet the requirements of both awards; however, the same period of service establishing one cannot be used to justify service eligibility for the other.





DISCUSSION AND CONCLUSIONS:

1.  The evidence shows that the applicant was promoted to the rank and pay grade of SSG/E-6 effective 1 April 2005.  Therefore, he is entitled to correction of item 4a of his DD Form 214, dated 6 June 2006 to show the entry "SSG"; item 4b to show the entry "E-6"; and item 12h to show the entry "2005 04 01" (1 April 2005).

2.  At the time of preparation of the applicant's DD Form 214, dated 6 June 2006, a typographical error occurred in the street name in item 19a and 19b.  The evidence clearly shows that the street name should be "Shooting Star" instead of "Shotting Star."  Therefore, he is entitled to correction of item 19a and 19b of his DD Form 214, dated 6 June 2006, to show the street address as "Shooting Star."

3.  The evidence shows that the applicant's unit was deployed in support of Operation Enduring Freedom and he served in Afghanistan from 11 February 2003 to 9 September 2003.  The applicant is therefore eligible for award of the Afghanistan Campaign Medal and the Global War on Terrorism Service Medal, and to have these medals added to his DD Form 214, dated 22 October 2003.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected: 

	a.  by showing the entry "SSG" in item 4a (Grade, Rate, or Rank) of his DD Form 214, dated 6 June 2006; 

	b.  by showing the entry "E-6" in item 4b (Pay Grade) of his DD Form 214, dated 6 June 2006;

c.  by showing the entry, 2005 04 01" in item 12h (Effective Date of Pay Grade) of his DD Form 214, dated 6 June 2006; 

	d.  by showing the entry, "Shooting Star" in item 19a (Mailing Address After Separation) and item 19b (Nearest Relative) of his DD Form 214, dated 6 June 2006; and 

	e.  by awarding the applicant the Afghanistan Campaign Medal and the Global War on Terrorism Service Medal, and adding these awards to item 13 (Decoration, Medals, Badges, Citation and Campaign Ribbons Awarded or Authorized) of his DD Form 214, dated 22 October 2003.
