THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant originally requested that the amount of Active Federal Commissioned Service (AFSC) be corrected. However, she later changed her request to ask that her constructive service credit be added to her service computation worksheet.
2. The applicant states that her service computation is accurate, but the entry for constructive service credit is missing.
3. The applicant provides a copy of her service computation worksheet, dated
1 May 2007, and a letter with a brief timeline of her medical and military career as a physician, dated 22 May 2007.
CONSIDERATION OF EVIDENCE:
1. Records show the applicant served on active duty as a Reserve commissioned officer for 7 years, 6 months and 22 days. The applicant had a break in service and was reappointed as a Reserve major and entered on active duty on 19 April 2003 as a lieutenant colonel.
2. The applicant is currently serving on active duty, assigned to Madigan Army Medical Center, Tacoma, Washington.
3. The service computation worksheet that the applicant submitted, in item
12 (Constructive Service for MC/DC) shows the entry "00 00 00."
4. During the research of this case the Board's staff contacted the AMEDD Branch at the Army Human Resources Command and was able to obtain a copy of the applicant's DA Form 5074-R (Record of Award of Entry Grade Credit (Medical and Dental Officers)), dated 4 April 2003. This form shows in Part III item 34 (Total Constructive Credit) 9 years, 10 months, and 8 days.
DISCUSSION AND CONCLUSIONS:
1. The applicant requests that her constructive service credit be added to her service computation worksheet.
2. Evidence shows that the applicant was awarded total constructive credit of
9 years, 10 months, and 8 days. Therefore, the applicant is entitled to correction of her records to show in item 12 of the service computation worksheet 9 years, 10 months, and 8 days.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief and to excuse failure to timely file. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing in item 12 of the applicant's service computation worksheet total constructive credit of 9 years, 10 months, and
8 days.
