                            RECORD OF PROCEEDINGS

      IN THE CASE OF:

      BOARD DATE: 14 June 2007
      DOCKET NUMBER: AR20070001497

I certify that hereinafter is recorded the true and complete record
of the proceedings of the Army Board for Correction of Military Records in
the case of the above-named individual.

| |Ms. Catherine C. Mitrano | |Director |
| |Mr. Luis Almodova | |Senior Analyst |

The following members, a quorum, were present:

| |Ms. Margaret K. Patterson | |Chairperson |
| |Mr. Ronald D. Gant | |Member |
| |Mr. Rowland C. Heflin | |Member |

The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, that her DD Form 214, Certificate of
Release or Discharge from Active Duty, be corrected to show the correct
amount of SGLI (Servicemembers Group Life Insurance) Coverage she had at
the time of her discharge.

2. The applicant states, in effect, the SGLI coverage on her certificate
of discharge is incorrect. She requests that it be changed to $250,000
from $10,000. Though her certificate of discharge is incorrect, she has
LES (Leave and Earnings) receipts from August 2003 through December 2003
which reflect SGLI deductions of $16.25 per month. The amount, $16.25, was
required for $250,000 coverage, as indicated on the LES receipts.

3. In support of her application, the applicant submits a letter of
support from the Shelter Legal Services Foundation, Inc., dated 22January
2007; a copy of her DD Form 214; and copies of her DFAS Form 702, Defense
Finance and Accounting Service Military Leave and Earnings Statement, for
the period August 2003 through December 2003.

CONSIDERATION OF EVIDENCE:

1. The applicant is requesting correction of an alleged error or injustice
that occurred on 10 December 2003, the date of her discharge. The
application submitted in this case is dated 10 November 2006, and was
received on 30 January 2007.

2. The applicant enlisted in the US Army Reserve, in the Delayed Entry
Program on 16 November 2002. She enlisted in the Regular Army on 14 July
2003. She was discharge from the Army, with an uncharacterized discharge,
under the provisions of Army Regulation (AR) 635-200, Chapter 11, for entry
level performance and conduct.

3. Item 10, (SGLI Coverage), of the DD Form 214, she was issued on her
discharge, shows she had $10,000.00 in SGLI coverage in effect when she was
discharged.

4. The LESs the applicant submitted in support of her request show $16.25
was being deducted each month from her basic pay for $250,000 SGLI
coverage.

5. AR 635-5 prescribes the separation documents that must be prepared for
Soldiers on retirement, discharge, release from active duty service, or
control of the Active Army. It establishes standardized policy for
preparing and distributing the DD Form 214. The instructions for the
completion of Item 10 are, "Enter the amount of Servicemembers Group Life
Insurance coverage, or 'X' for none."

DISCUSSION AND CONCLUSIONS:

The evidence submitted by the applicant supports her contention that the
entry is incorrect. She is therefore entitled to a correction to her DD
Form 214, Item 10, to show she had $250,000 in SGLI coverage in force on
the date of her discharge from the Army.

BOARD VOTE:

_RCH ___ _MKP___ _RDG___ GRANT FULL RELIEF

________ ________ ________ GRANT PARTIAL RELIEF

________ ________ ________ GRANT FORMAL HEARING

________ ________ ________ DENY APPLICATION

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
correcting the applicant's DD Form 214, Item 10, to show she had $250,000
in SGLI coverage in force on the date of her discharge from the Army.
