THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that his deployment to Kuwait be annotated on his separation document (DD Form 214).
2. The applicant states, in effect, that he was deployed to Kuwait in support of Operation Iraqi Freedom on 28 December 2004. He further states that his
DD Form 214 does not reflect his deployment in Item 18 (Remarks), and he is now requesting his records be corrected to document his deployed service in Kuwait.
3. In support of his application, the applicant provides Detachment A, 203rd Personnel Services Battalion, Fort Wainwright, Alaska, Orders Number 307-305, dated 2 November 2004, with an attached personnel roster.
CONSIDERATION OF EVIDENCE:
1. The applicants record shows he enlisted in the Regular Army (RA) and entered active duty on 7 February 1995. He was trained in, awarded, and served in military occupational specialty (MOS) 92Y (Unit Supply Specialist), and the highest rank he attained while serving on active duty was sergeant.
2. Detachment A, 203rd Personnel Services Battalion, Fort Wainwright, Alaska, Orders Number 307-305, dated 2 November 2004, directed the deployment of the applicants unit to Kuwait, in support of Operation Iraqi Freedom, on or about 22 December 2004. A roster of individuals deployed with the unit, which included the applicant, was attached to these orders.
3. The applicant's Official Military Personnel File (OMPF) contains an Army Achievement (AAM) Certificate, dated 19 January 2006. It shows the applicant received the AAM for his meritorious service while assigned as the unit supply sergeant for Aviation Task Force Kuwait, during the period 28 December 2004 through 13 August 2005.
4. In connection with the processing of this case, a member of the Board staff reviewed the applicant's Defense Finance and Accounting Service (DFAS) pay record, which confirms the applicant received imminent danger pay for Kuwait from December 2004 through February 2005.
5. On 18 June 2006, the applicant was honorably separated by reason of completion of required active service under the provisions of chapter 4,
Army Regulation 635-200. The DD Form 214 he was issued at the time confirms he completed a total of 11 years and 24 days of active military service. Item
12f (Foreign Service) shows he completed 9 years, 2 months and 3 days of overseas service and Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) shows he earned the following awards during his tenure on active duty: AAM (4th Award), Army Good Conduct Medal, National Defense Service Medal (2nd Award), Global War on Terrorism Service Medal (GWOTSM), Global War on Terrorism Expeditionary Medal (GWOTEM), Noncommissioned Officer Professional Development Ribbon Numeral 2, and Army Service Ribbon.

Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It also establishes standardized policy for preparing and distributing the DD Form 214. Chapter 2 contains guidance on the preparation of the DD Form 214. It states, in pertinent part, that the source documents for entering information on the DD Form 214 will be the Personnel Qualification Record (PQR), Enlisted/Officer Record Brief (ERB/ORB), or any other document authorized for filing in the OMPF.
7. Paragraph 2-4 of the separation documents regulation contains item-by-item instructions for completing the DD Form 214. The instructions for Item 13 show that the awards entered will be for all periods of service and instructs the preparing official to check the Soldiers service records for the validity of awards. It also instructs that the awards will be entered in the order of merit they are listed in the Armys awards regulation.
8. The instructions contained in the separation documents regulation for completion of Item 18 (Remarks) state, in pertinent part, that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the following entry will be made: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that he served overseas in Kuwait was carefully considered and found to have merit. The evidence of record confirms he served in Kuwait from 28 December 2004 through February 2005, as corroborated by his DFAS pay record, which confirms he received imminent danger pay for this period.
2. By regulation, the deployed service of an active duty Soldier should be documented with an entry in Item 18 of the DD Form 214. In this case,
the following entry should be added to Item 18 of the applicant's DD Form 214
to document his service in Kuwait: "SERVICE IN KUWAIT FROM
20041228-20050228".

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by
amending Item 18 of his 18 June 2006 DD Form 214 by adding the entry
"SERVICE IN KUWAIT FROM 20041228-20050228" and by providing him a correction to his separation document that includes this change.
