	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show the third award of the Army Achievement Medal.  He also requests that item 18 (Remarks) be corrected to show he received $51575.40 not $515755.40 in disability severance pay.

2.  The applicant states, in effect, that he received the third award of the Army Achievement Medal at his home of record after his discharge and that the entry in item 18 is a typing error.

3.  The applicant provides a copy of his DD Form 214; a Leave and Earnings Statement (LES); and orders and an award certificate for the third award of the Army Achievement Medal. 

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted on 22 August 2000 and served as a wheeled vehicle mechanic.  On 15 May 2006, he was honorably discharged by reason of physical disability with severance pay.

2.  Item 13 on the applicant’s DD Form 214 does not show the third award of the Army Achievement Medal.  Item 18 on his DD Form 214 shows the entry, “DISABILITY SEVERANCE PAY -- $515755.40.”

3.  Headquarters, 710th Brigade Support Battalion Permanent Orders Number 121-01, dated 1 May 2006, show the applicant received the third award of the Army Achievement Medal for the period 15 November 2004 to 15 January 2006.

4.  The applicant provided a LES covering the period 1-15 May 2006 which shows he received $51575.40 in disability severance pay.  

DISCUSSION AND CONCLUSIONS:

1.  Orders show the applicant received the third award of the Army Achievement Medal.

2.  The LES provided by the applicant which shows he received $51575.40 in disability severance pay is accepted as sufficient evidence on which to amend item 18 on his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  amending item 13 on his DD Form 214 to add the third award of the Army Achievement Medal;   

	b.  deleting the entry, “DISABILITY SEVERANCE PAY -- $515755.40.” in item 18 on his DD Form 214; and

	c.  adding the entry, “DISABILITY SEVERANCE PAY -- $51575.40” in item 18 on his DD Form 214.
