      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of
Release or Discharge from Active Duty) to show two awards of the Army
Commendation Medal, two awards of the Army Achievement Medal, and the Army
Good Conduct Medal.

2.  The applicant states that these awards are missing from his records due
to inattentive clerical errors during demobilization from Operation Iraqi
Freedom deployment.

3.  The applicant provides his DD Form 214 (Worksheet); three DA Forms 638
(Recommendation for Award); and two award certificates.

CONSIDERATION OF EVIDENCE:

1.  The applicant served in support of Operation Iraqi Freedom.

2.  The applicant enlisted in the Regular Army on 19 August 1997.  He
completed the required training and was awarded military occupational
specialty 63T (Bradley Fighting Vehicle Systems Mechanic).  He was promoted
to specialist on 20 February 1999.

3.  The applicant’s DA Form 638, dated 26 April 2000, shows he was
recommended for award of the Army Achievement Medal.  Block 8 (Previous
Awards) on this document shows he received one previous award of the Army
Achievement Medal.

4.  Headquarters and Headquarters Battery, 1st Battalion, 6th Field
Artillery Permanent Order Number 139-15, dated 18 May 2000, awarded the
applicant the Army Achievement Medal for meritorious achievement from 15
November 1999 to 15 June 2000.

5.  Headquarters, 3rd Brigade Permanent Order Number 121-28, dated 30 April
2000, awarded the applicant the Army Commendation Medal as an end of tour
award for meritorious service during the period 18 August 1997 to 18 August
2000.

6.  The applicant was released from active duty on 18 August 2000 in the
rank of specialist.

7.  His DD Form 214 for the period ending 18 August 2000 shows the Army
Service Ribbon, the Overseas Service Ribbon, and the Kosovo Campaign Medal
as authorized awards.

8.  The applicant enlisted in the Army National Guard on 6 March 2001.  He
was ordered to active duty on 28 June 2004.

9.  The applicant’s DA Form 638, dated 10 July 2005, shows he was
recommended for the Army Commendation Medal.  Block 8 on this document
shows he received one previous award of the Army Achievement Medal.

10.  Headquarters, 116th Brigade Combat Team Permanent Order Number
218-004, dated 6 August 2005, awarded the applicant [a second award of] the
Army Commendation Medal for meritorious service from 1 December 2004 to
10 November 2005.

11.  The applicant provided an award certificate which shows he was awarded
the Army Good Conduct Medal for the period 28 June 2004 to 10 November
2005.

12.  The applicant was released from active duty on 22 November 2005 and
was transferred back to his National Guard unit on the following date.

13.  His DD Form 214 for the period ending 22 November 2005 shows the
National Defense Service Medal, the Army Service Ribbon, the Armed Forces
Reserve Medal with “M” Device, the Global War on Terrorism Service Medal,
and the Iraq Campaign Medal as authorized awards.

14.  He is currently serving in the Army National Guard in the rank of
specialist.

15.  Army Regulation 600-8-22 (Military Awards) states, in pertinent part,
that the bronze oak leaf cluster is awarded to denote the second and
succeeding awards of certain decorations, including the Army Commendation
Medal.

DISCUSSION AND CONCLUSIONS:

1.  Permanent orders show the applicant was awarded the Army Achievement
Medal for meritorious achievement from 15 November 1999 to 15 June 2000.

2.  Permanent orders show the applicant was awarded the Army Commendation
Medal as an end of tour award for meritorious service during the period
18 August 1997 to 18 August 2000.

3.  The applicant enlisted in the Army National Guard on 6 March 2001 and
served on active duty from 28 June 2004 to 22 November 2005.  Permanent
orders show he was awarded a second award of the Army Commendation Medal
while he was on active duty.  Therefore, it would be appropriate to amend
his DD Form 214 for the period ending 22 November 2005 to reflect the Army
Achievement Medal and two awards of the Army Commendation Medal.

4.  Although the applicant’s DA Form 638, dated 26 April 2000, indicates he
received a previous award of the Army Achievement Medal, there are no
orders which verify he was awarded two awards of the Army Achievement
Medal.  Therefore, there is insufficient evidence on which to show he was
awarded two awards of the Army Achievement Medal.

5.  The applicant provided an award certificate which shows he was awarded
the Army Good Conduct Medal for the period 28 June 2004 to 10 November
2005.  This certificate is accepted as sufficient evidence to amend his DD
Form 214 for the period ending 22 November 2005 to show the Army Good
Conduct Medal.

6.  The applicant’s DD Form 214 for the period ending 18 August 2000 shows
the Army Service Ribbon, the Overseas Service Ribbon, and the Kosovo
Campaign Medal.  However, his DD Form 214 for the period ending 22 November
2005 failed to list the Overseas Service Ribbon and the Kosovo Campaign
Medal from his prior service.  Therefore, it would be appropriate to amend
his DD Form 214 for the period ending 22 November 2005 to show the Overseas
Service Ribbon and the Kosovo Campaign Medal.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by amending his DD Form 214 for the period ending 22
November 2005 to show the Army Achievement Medal, two awards of Army
Commendation Medal, the Army Good Conduct Medal, the Overseas Service
Ribbon, and the Kosovo Campaign Medal.

2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
two awards of the Army Achievement Medal.
