The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that her records be corrected to show she served
in Iraq from 2 April 2003 through 31 March 2004.
2. The applicant states the wrong dates were typed on her DD Form 214
(Certificate of Release or Discharge from Active Duty) based on the dates
on an award and not the actual dates she served. She relates she served in
Iraq for a year.
3. The applicant provides copies of her DD Form 214 and a finance printout
showing service in a combat zone.
CONSIDERATION OF EVIDENCE:
1. The records show the applicant enlisted under the Delayed Entry Program
on 13 December 1991 and entered active duty on 28 July 1992. She remained
on active duty until she was honorably released from active duty on 24
August 2005.
2. The applicant's Official Military Personnel File contains no
documentation later than the award of the Good Conduct Medal in October
2002.
3. The DA Form 2-1 (Personnel Qualification Record) shows 56 months of
service in Germany and 36 months in Alaska, a total of 7 years and 8
months.
4. The DD Form 214 shows she had 13 years and 27 days of creditable
service with 7 years, 11 months, and 2 days of foreign service. Item 18
(Remarks indicates service in Iraq for the period 1 April 2003 to 10 July
2003, 3 months and 7 days.
5. The financial record provided to the applicant by Master Sergeant R.
K____, Financial Manager, indicates the applicant served in a combat zone
from 1 April 2003 through 31 March 2004 and received imminent danger pay
for this period.
6. Army Regulation 635-5 (Separation Documents) prescribes the separation
documents prepared for Soldiers upon retirement, discharge, or release from
active military service or control of the Army. It establishes
standardized policy for the preparation of the DD Form 214. It states
that, for an active duty Soldier deployed with his or her unit during their
continuous period of active service, the
statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for
example, YYYYMMDD - YYYYMMDD)" will be entered in item 18 (Remarks).
6. It also states that, for Item 12 (Record of Service) f (Foreign
Service), from the Enlisted Record Brief (ERB)/Officer Record Brief (ORB),
enter the total amount of foreign service completed during the period
covered in block 12c (Net Active Service This Period).
DISCUSSION AND CONCLUSIONS:
1. Since the DA Form 2-1 is incomplete, the entries taken from this form
for notation on the DD Form 214 are also incomplete or incorrect.
2. Based on the applicant's statement and the financial record, it appears
that the applicant did serve in Iraq for a period longer than what is
listed on the DD Form 214.
3. Although the specific dates of the applicant's service in Iraq can not
be fully validated, it appears that she served in Iraq for at least the
period 1 April 2003 through 31 March 2004 and her records should be
corrected to reflect this period of foreign service.
4. Additionally, item 12f on the DD Form 214 should be corrected to show
an additional year of foreign service for a total of 8 years, 11 months,
and 2 days of foreign service.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
showing the applicant served in Iraq during the period 1 April 2003 through
31 March 2004 and that she had 8 years, 11 months, and 2 days of foreign
service.
