The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of Item 12.d (Total Prior
Active Service) and Item 12.e (Total Prior Inactive Service), of his DD
Form 214, Certificate of Release or Discharge from Active Duty.
2. The applicant states, in effect, that when he was released from active
duty, his DD Form 214 was not completed because of a lack of prior active
and inactive service.
3. The applicant provides a copy of his DD Forms 214, with effective
dates of 11 April 2003, 24 June 1991, 28 August 1988, and 22 July 1987; a
copy of his Honorable Discharge certificate from the United States Marine
Corps, dated 7 September 1999; a copy of his DD Form 4 (page 1 and page
2), dated 9 June 2000; and a copy of his orders which ordered him to
active duty on 17 June 2002, in support of his request.
CONSIDERATION OF EVIDENCE:
1. The applicant’s military records show that he enlisted in the Army
Reserve, for 6 years, in the rank and pay grade of Sergeant, E-5, on 9
June 2000. The applicant had prior active and inactive service in the
United States Marine Corps Reserve.
2. On 17 June 2002, the applicant was ordered to active duty for 365
days for the purpose of supporting Operation Enduring Freedom.
3. The applicant deployed and served in Afghanistan from 2 August 2002
through 18 March 2003. The applicant's DD Form 214 shows that he was
released from active duty and returned to his unit, the 377th Military
Police Company, Cincinnati, Ohio, on 11 April 2003, in the rank and pay
grade Staff Sergeant, E-6.
4. Item 12.d. AND 12.e., of the applicant's DD Form 214, shows the
entry, "SEE BLOCK #18".
5. Item (Block) 18, of the applicant's DD Form 214, shows an entry, "DD
Form 215 will be issued to provide missing information".
6. A review and a calculation of the applicant's service revealed that
the applicant had a total of 13 years, 7 months, and 2 days prior
inactive service and, 11 months and 7 days prior active service before he
entered active duty on 17 June 2002.
7. AR 635-5 prescribes the separation documents that must be prepared for
Soldiers on retirement, discharge, release from active duty service, or
control of the Active Army. It establishes standardized policy for
preparing and distributing the DD Form 214. Chapter 2, paragraph 2-4
provides specific instruction for the completion of the DD Form 214,
including Item 12.d. and e. Item 12.d. will show the total amount of prior
active service, less lost time, if any. Item 12.e. will show the total
amount of prior inactive service, less lost time, if any.
DISCUSSION AND CONCLUSIONS:
1. The DD Form 214 that was issued to the applicant incorrectly shows that
he entered active duty on 17 June 2002. The applicant was released from
active duty on 11 April 2003. Item 12.d. and 12.e. could not be completed
due to a lack of date pertinent to the applicant's prior active and
inactive service. Recognizing this, an entry was made in these date items,
"See Block #18."
2. Item (Block) 18 states that, "DD Form 215 will be issued to provide
missing information."
3. A review and a calculation of the applicant's service revealed that
the applicant had a total of 13 years, 7 months, and 2 days prior
inactive service and, 11 months and 7 days prior active service before he
entered active duty on 17 June 2002.
4. Based on the review and calculation, the entries shown in Items 12.d.
and 12.e. will be deleted and the following will be entered in Item 12.d.,
"0000 years / 11 months / 07 days". The following will be entered in Item
12.e., "0013 years / 07 months / 02 days".

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by:
a. deleting the entry "SEE BLOCK #18" from Item 12.d. and entering
"0000 / 11 / 07", in its place to show the correct amount of prior active
service the applicant had to his credit; and
b. deleting the entry, "SEE BLOCK #18" from Item 12.e. and entering
0013 / 07 / 02, in its place to show the correct amount of prior inactive
service the applicant had served before the date of his entry on active
duty.
