if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, correction of item 18 (Remarks), of
his DD Form 214 (Certificate of Release or Discharge from Active Duty), to
show his service in Iraq from 1 May to 23 December 2003.

2. The applicant states, in effect, that item 18, of his DD Form 214, is

separation briefing, he was informed of the error.

3. The applicant provides a copy of his DD Form 214 and several documents
from his Official Military Personnel File (OMPF), in support of his
request.

1. The applicant's military records show he entered active duty (AD) on
2 August 2001, as an armor crewman (19K). He was advanced to pay grade E-4
effective 1 July 2005. He continued to serve until he was released from AD
on 1 August 2005. He was transferred to the U. S. Army Reserve (USAR)

2. Item 18 (Remarks), of the applicant’s DD Form 214, does not show his
wartime service.

3. The applicant provides a copy of Headquarters, 1st Armored Division
Orders 106-28, dated 16 April 2003, which shows that his unit, the 1st
Battalion, 35th Armored Regiment, 1st Armored Division, was deployed
in support of Operation Enduring Freedom, effective 26 April 2003 to
25 April 2004, not to exceed 365 days.

4. The applicant provides a copy of Memorandum for Record, Subject: Proof
of Service, prepared by the personnel officer, of his unit, dated
15 November 2003. The memorandum certified that the applicant served with
Company C, 1st Battalion, 35th Armored Regiment, in support of the 1st
Armored Division Task Force during Operation Iraqi Freedom from 1 May to
23 December 2003.

5. Army Regulation 635-5 establishes the standardized policy for preparing
and distributing the DD Form 214. In pertinent part, it directs that
foreign service performed during the period covered by the DD Form 214 will
be entered in item
12f. The regulation also states that the amount of foreign service will be
taken from the Soldier’s Enlisted Qualification Record or, if necessary,
verified from the Soldier’s Military Personnel Records Jacket. However,
for active duty Soldiers deployed with their unit during their continuous
