                            RECORD OF PROCEEDINGS

      IN THE CASE OF:

      BOARD DATE: 8 December 2005
      DOCKET NUMBER: AR20050006521

I certify that hereinafter is recorded the true and complete record
of the proceedings of the Army Board for Correction of Military Records in
the case of the above-named individual.

| |Mr. Carl W. S. Chun | |Director |
| |Mr. Joseph A. Adriance | |Analyst |

The following members, a quorum, were present:

| |Mr. Richard T. Dunbar | |Chairperson |
| |Mr. James B. Gunlicks | |Member |
| |Mr. Scott W. Faught | |Member |

The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, that Item 12f (Foreign Service) of
his separation document (DD Form 214) be corrected to reflect his service
in Iraq.

2. The applicant states, in effect, Item 12f of his DD Form 214 should be
corrected to reflect his Iraq service consistent with the Item 18 (Remarks)
entry that confirms he served in Kuwait and Iraq from 26 February through
31 December 2004.

3. The applicant provides a copy of his DD Form 214 in support of his
application.

CONSIDERATION OF EVIDENCE:

1. The applicant’s record shows that while serving in the Army National
Guard (ARNG), the applicant was ordered to active duty on 1 October 2003.
He held and served in military occupational specialty (MOS) 11B
(Infantryman).

2. The applicant’s record also shows he was deployed to and served in
Kuwait and Iraq from 26 February through 31 December 2004, and that he was
promoted to specialist (SPC) on 25 August 2002.

2. On 4 February 2005, the applicant was honorably released from active
duty and returned to his ARNG unit after completing 1 year, 4 months, and 4
days of active military service during the period. Item 12f of the DD Form
214 he was issued contains the entry “0000 00 00”, which indicates he
completed no foreign service during the period.

3. Item 18 of the applicant’s DD Form 214 contains the following entry
“Service in Kuwait/Iraq: 20040226-20041231”, which confirms he served
overseas in Kuwait and Iraq for the period indicated.

4. Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that must be prepared for soldiers on retirement, discharge,
release from active duty service, or control of the Active Army. It also
establishes standardized policy for preparing and distributing the DD Form
214. Chapter 2 contains item-by-item instructions for completing the DD
Form 214. The instructions for Item 12f specify that the total amount of
foreign service completed during the period will be entered.

DISCUSSION AND CONCLUSIONS:

The evidence of record confirms the applicant served overseas in Kuwait and
Iraq for 10 months and 5 days, from 26 February through 31 December 2004.
Thus, it would be appropriate to add this foreign service to Item 12f of
his separation document at this time.

BOARD VOTE:

___RTD _ ___JBG _ ___SWF GRANT FULL RELIEF

________ ________ ________ GRANT PARTIAL RELIEF

________ ________ ________ GRANT FORMAL HEARING

________ ________ ________ DENY APPLICATION

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending Item 12f of his DD Form 214 by deleting the current entry and
replacing it with the entry “00 10 05”; and by providing him a correction
to his separation document that includes this change.
