The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, that her DD Form 214, Certificate of
Release or Discharge from Active Duty, be corrected to include all her
service time.

2. The applicant states, in effect, that upon her release from airborne
school she was not given a DD Form 214, to include the time between the end
of her officer candidate school and the beginning of airborne school.

3. In a second application submitted to the Board, she states, in effect,
that her active duty time was not corrected on the DD Form 214 that was
prepared after completion of officer basic course and she asks to have her
intelligence officer training put on the DD Form 214 for her officer basic
course.

4. The applicant provides a collection of thirty-nine documents related
to her request, including a copy of her two DD Forms 214; a copy of the
DA Form 1506, Statement of Service for Computation of Length of Service
for Pay Purposes, dated 5 December 2003; a copy of a DD Form 220, Active
Duty Report, dated 12 March 2003; and a copy of her DD Form 1996/1-4,
Record of Military Processing – Armed Forces of the United States, dated
22 November 2002; and a copy of her DD Form 4/1 and 2,
Enlistment/Reenlistment Document, Armed Forces of the United States,
dated 22 November 2002; in support of her request.

CONSIDERATION OF EVIDENCE:

1. The applicant's records show she enlisted in the US Army Reserve for 8
years, in the pay grade E-4, with no prior military service.

2. On 2 January 2003, the applicant was ordered to active duty to undergo
basic training. She completed this training and was released from active
duty for training on 13 March 2003. The applicant was provided a DD Form
220 to cover this period of service.

3. On 7 September 2003, the applicant was ordered to active duty to attend
officer candidate school. She successfully completed this training and on
10 December 2003, she was given a DD Form 214 showing she was honorably
released from active duty, at the completion of her required service and,
was transferred to the US Army Reserve Control Group (Individual Ready
Reserve).

4. On 18 December 2003, Orders 324508, published by the Military Entrance
and Processing Station (MEPS), Denver, Colorado, on 2 September 2003, were
amended. The period of the applicant's active duty for training period
that read: ADT Period: 08 September 2003 to 17 December 2003, was amended
to read: ADT Period: 08 September 2003 to 23 January 2004.

5. Item 12 (Record of Service), subparagraph d. (Total Prior Active
Service), of the applicant's DD Form 214, shows the entry: 0000 years / 02
months / 12 days. This entry accounts only for the time up to 10 December
2003.

6. The applicant enrolled in and completed Airborne Course, Class 09-04,
at Fort Benning, Georgia, on 23 January 2004. She was awarded the
Parachutist Badge. After completion of the course, she was honorably
released from active duty for training and was transferred to the US Army
Reserve Control Group (Individual Ready Reserve), with a Reserve obligation
until 21 November 2010.

7. Item 14 (Military Education), of the applicant's DD Form 214, shows the
entries: Officer Candidate School, 14 weeks, December 2003//Basic Combat
Training, 9 weeks, March 2003. The applicant completed the 3-week Airborne
Course, during the period covered by this DD Form 214.

8. Administrative errors made in the preparation of the applicant's DD
Form 214 for her second period of active duty service were carried over to
her third period of active duty service and resulted in erroneous entries
on the DD Form 214 with an end date of 24 January 2005.

9. Item 12.d., of the applicant's DD Form 214, should have shown a total
of 06 months and 29 days total prior active service, as opposed to the
currently shown 05 months and 16 days.

10. Item 12.e., of the applicant's DD Form 214, should have shown a total
of 0001 year, 02 months, and 09 days total prior inactive service, as
opposed to the 0001 year, 03 months and 22 days now shown.

DISCUSSION AND CONCLUSIONS:

1. The applicant was ordered to active duty to complete basic training.
She served 01 month and 12 days active duty. At completion of this
training she was released back to her unit.

2. The applicant was ordered to active duty to undergo officer candidate
training. The applicant's originally published active duty for training
orders were to expire on 10 December 2003.

3. The applicant's active duty for training period was extended to 23
January 2004 to allow her to undergo airborne training following her
completion of officer candidate school.

4. The applicant's DD Form 214 now showing a through date of 10 December
2003 should be corrected as follows:

a.) Item 12.b. (Separation Date This Period) should be changed to:
2004 / 01 / 23. b.); and

b.) Item 12.c. (Net Active Service This Period) should be changed to:
0000 years / 04 months / 17 days.

5. The applicant completed Airborne Basic Course, Class 09-04, during this
period and was awarded the Parachutist Badge. Item 13, of this DD Form
214, should be corrected to show the Parachutist Badge. Item 14, of this
DD Form 214, should be corrected to add the "Airborne Basic Course, 3
weeks, Jan 2004."

6. Errors made in the preparation of the applicant's DD Form 214 for her
officer candidate school and airborne course training period, ending 23
January 2004, overflowed to the DD Form 214 that was prepared for her on
24 January 2005, when she completed her officer basic course. Therefore,
correction of this DD Form 214 is required as follows:

      a. Item 12.d. (Total Prior Active Service) should be changed to:
0000 years / 06 months / 29 days.

      b. Item 12.e. (Total Prior Inactive Service) should be changed to:
0001 year / 02 months / 09 days.
