The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that the fourth digit of her service number
shown on the Certification of Military Service, dated 8 March 2005, be
corrected to show “0” instead of “6.”
2. The applicant states her service number is incorrect on this document.
3. The applicant provides a copy of her Certification of Military Service.
CONSIDERATION OF EVIDENCE:
1. The applicant’s military records are not available to the Board for
review. A fire destroyed approximately 18 million service members’ records
at the National Personnel Records Center in 1973. It is believed that the
applicant’s records were lost or destroyed in that fire. However, there
were sufficient documents remaining in a reconstructed record for the Board
to conduct a fair and impartial review of this case.
2. The applicant was a member of the Regular Army from 20 August 1954 to
16 March 1956. She was a specialist third class at the time of her
honorable discharge on 16 March 1956.
3. The applicant’s Certification of Military Service, dated 8 March 2005,
shows the fourth digit of her service number is “6.”
4. A Morning Report, dated 25 August 1954, shows the applicant’s fourth
digit of her service number is “0.”
5. The applicant’s discharge orders, dated 14 March 1956, show the fourth
digit of her service number is “0.”
DISCUSSION AND CONCLUSIONS:
Contemporaneous records show the applicant’s fourth digit of her service
number is “0.” Therefore, it would be appropriate to correct the fourth
digit of her service number on the Certification of Military Service, dated
8 March 2005, to show “0.”

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending the fourth digit of her service number to show “0” on the
Certification of Military Service, dated 8 March 2005.

The applicant’s military records are not available to the Board for review. The report of separation and death certificate both show the same date of birth for the FSM. The FSM’s report of separation shows his first name as “Ervis;” however, his discharge order shows “Eris.” The FSM himself, in 1997 when requesting a copy of his report of separation, printed and signed his first name as “Eris.” 2.

Evidence of record shows the applicant was honorably discharged on 3 November 1954 for immediate reenlistment.
