         I certify that hereinafter is recorded the true and complete record of the proceedings of the Army Board for Correction of Military Records in the case of the above-named individual. A quorum was present during the further consideration and deliberation. The findings appearing in proceedings dated 6 November 2003 were affirmed. The following additional findings, conclusions, and recommendations were adopted by the Board.

CONSIDERATION OF ADDITIONAL EVIDENCE:

19. On 9 February 2004, the Chief, Military Awards Branch at the U.S. Army Human Resources Command recommended that additional administrative action be taken in the applicant's records in regard to the decision of the Army Board for Correction of Military Records (ABCMR) in Docket Number AR2003089372, dated 6 November 2003. Based on the award entries contained in the applicant's DD Forms 214, it was determined that administrative action to amend the applicant's DD Form 214 for the period ending 24 September 1969 to show the awards reflected in paragraph 2 in the recommendation section was not required. Also, based on review of the applicant's service personnel records, it was recommended that item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on the applicant's DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending
31 December 1986 be amended by deleting the Army Commendation Medal and adding the Purple Heart, the Army Commendation Medal with Second Oak Leaf Cluster, the Good Conduct Medal (7th Award), the Meritorious Unit Commendation, and the Valorous Unit Award.

20. There is no requirement to amend the applicant's DD Form 214 for the period ending 24 September 1969.

21. Paragraph 3 in the recommendation section recommends that the applicant's DD Form 214 for the period ending 31 March 1986 be corrected to show award of the Purple Heart, the Valorous Unit Award, and the Meritorious Unit Commendation. The correct effective date for this DD Form 214 is 31 December 1986.

ADDITIONAL DISCUSSION AND CONCLUSIONS:

7. At the time of the decision of the ABCMR in Docket Number AR2003089372, dated 6 November 2003, it was the intent of the ABCMR to make the applicant’s record as administratively correct as it should properly have been at the time.

8. There is no requirement to correct the applicant's DD Form 214 for the period ending 24 September 1969 in the ABCMR's decision in Docket Number AR2003089372, dated 6 November 2003.

9. The effective date of the applicant's retirement DD Form 214 was incorrectly shown as 31 March 1986 instead of 31 December 1986 in paragraph 3 in the recommendation section of the Proceedings, AR2003089372, dated 6 November 2003.

10. Review of the applicant's service personnel records show that he is entitled to have his DD Form 214 for the period ending 31 December 1986 amended to add award of the Purple Heart, the Army Commendation Medal with Second Oak Leaf Cluster, the Good Conduct Medal (7th Award), the Meritorious Unit Commendation, and the Valorous Unit Award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant amendment of the decision of the Army Board for Correction of Military Records set forth in Docket Number AR2003089372, dated 6 November 2003, by:

         a. deleting paragraphs 2 and 3 in the recommendation section of the Proceedings; and

         b. correcting paragraph 2 to state, "That the applicant's DD Form 214 for the period ending 31 December 1986 be corrected by:

a. deleting the entry, "Army Commendation Medal" in item 13; and

                  b. adding the entries, "Purple Heart," "Army Commendation Medal with Second Oak Leaf Cluster," "Good Conduct Medal (7th Award)," "Meritorious Unit Commendation," and "Valorous Unit Award" in item 13 on this DD Form 214.
