The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that a DA Form 638 (Recommendation for Award),
be corrected to show the correct Social Security Number (SSN).
2. The applicant states that a DA Form 638, dated 23 January 1998, should
be corrected to show his SSN as 164-XX-XXX instead of 194-XX-XXX. The
applicant further states that he would like to submit this award for
inclusion in his Navy service record.
3. The applicant provides:
a. A copy of a DA Form 638, dated 23 January 1998.
b. Orders Number 174-5, Headquarters, 81st Regional Support Command,
dated 23 June 1998.
c. A copy of his military identification card for the U.S. Navy.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the United States Air Force (USAF) on 11
August 1988. He was honorably discharged in the rank of Senior Airman on
10 August 1992. The applicant enlisted in the United States Army Reserve
(USAR) on
24 July 1997 for a period of 6 years. The applicant is currently serving
in the United States Navy.
2. The applicant’s DD Form 214 (Certificate of Release or Discharge from
Active Duty) for his USAF service shows SSN number as 164-XX-XXX.
3. The applicant’s DD Form 4/1 (Enlistment/Reenlistment Document) shows
his SSN as 164-XX-XXX.
4. The applicant provided a copy of Orders Number 174-5, Headquarters,
81st Regional Support Command, dated 23 June 1998. These orders show the
applicant was discharged from the USAR effective 23 June 1998. His SSN is
listed as 164-XX-XXX.
5. The majority of the documents in the applicant’s Official Military
Personnel File (OMPF) including medical records, consistently show his SSN
as 164-XX-XXX.
6. The applicant provided a DA Form 638 which shows he was awarded the
Army Commendation Medal on 18 March 1998 for outstanding service with the
982nd Replacement Company, Mobile, Alabama. This document shows his SSN as
194-XX-XXX. No other documents in the applicant’s OMPF show this SSN.
DISCUSSION AND CONCLUSIONS:
1. For historical purposes, the Army has an interest in maintaining the
accuracy of its records. The data and information contained in those
records should actually reflect the conditions and circumstances that
existed at the time the records were created. In the absence of a showing
of material error or injustice, this Board is reluctant to recommend that
those records be changed.
2. The SSN shown on the DA Form 638 appears to be a typographical error.
All documents in the applicant’s OMPF, including the applicant’s DD Form
214 with an effective date of 10 August 1992, shows his SSN as 164-XX-XXX.
The only exception is the DA Form 638 dated 23 January 1998 which
incorrectly shows his SSN as 194-XX-XXX. Therefore, there is sufficient
evidence upon which to base amending the DA Form 638 to show the correct
SSN.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that the DA
Form 638, dated 23 January 1998 awarding the applicant the Army
Commendation Medal be amended to show his correct SSN 164-XX-XXX.
