THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that Item 12d (Total Prior Active
Service) of his 25 June 2004 separation document (DD Form 214) be corrected
to reflect his total prior active duty service.
2. The applicant states, in effect, that his total active duty service is
correctly reflected in previous DD Forms 214 he was issued on 18 October
2002 and
10 July 2003.
3. The applicant provides three DD Forms 214 in support of his
application.
CONSIDERATION OF EVIDENCE:
1. The applicant is a major (MAJ) serving as an individual mobilization
augmentee (IMA) in the United States Army Reserve (USAR).
2. On 18 October 2002, the applicant was issued a DD Form 214 upon his
completion of an active duty tour. This document confirms he completed
11 months and 6 days of active duty service during the period covered by
the
DD Form 214. It also indicates that as of that date, he had completed 10
years, 10 months and 10 days of prior active duty service.
3. On 10 July 2003, the applicant was issued a DD Form 214 upon his
completion of an active duty tour. This document shows he completed 8
months and 22 days of active duty service during the period covered by the
report. It also indicates he had completed a total of 11 years, 9 months
and 16 days of prior active duty service.
4. On 25 June 2003, the applicant was honorably separated upon completion
of active duty for training. The DD Form 214 he was issued shows he
completed 11 months and
15 days of active military service during the period covered by the report.
Item 12d (Total Prior Active Service) contains the entry “SEE BLOCK 18”.
Item 18 (Remarks) contains an entry indicating he was separated on
temporary records and that a correction to the separation document would be
issued to provide missing information.
5. Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that must be prepared for Soldiers on retirement, discharge,
release from active duty service, or control of the Active Army. It also
establishes standardized policy for preparing and distributing the DD Form
214.
6. Chapter 2 of the separation documents regulation contains guidance on
the preparation and distribution of the DD Form 214. It states, in
pertinent part, that a DD Form 214 will be prepared on all Reserve
component (RC) Soldiers completing 90 days or more of continuous active
duty for training (ADT),
Full-Time National Guard Duty (FTNGD), active duty for special work (ADSW),
temporary tours of active duty (TTAD), or Active Guard Reserve (AGR)
service.
7. Paragraph 2-4 of the separation documents regulation contains
instructions on the preparation of the DD Form 214. It states, in
pertinent part, that the entry in Item 12d will be the total prior active
duty service as documented on previously issued DD Forms 214.
DISCUSSION AND CONCLUSIONS:
1. The applicant’s contention that his 25 June 2004 DD Form 214 does not
properly account for all his active duty service was carefully considered
and found to have merit. By regulation, Item 12d of the DD Form 214 will
be the total prior active duty service completed, as documented on
previously issued
DD Forms 214.
2. The evidence of record in this case includes two DD Forms 214 issued to
the applicant for periods of active duty service completed prior the active
duty tour of duty that is documented on his 25 June 2004 separation
document. These
DD Forms 214 confirm the applicant completed a total of 12 years, 6 months
and 8 days of prior active duty service. Therefore, it would be
appropriate to correct Item 12d of his 25 June 2004 DD Form 214 to reflect
this total prior active duty service.
3. It was also noted that there were additional items on the applicant’s
25 June 2004 that were not completed due to the lack of records. However,
there were also insufficient records provided for the Board review that
would allow for correction of these additional items. Therefore, if the
applicant wishes to pursue further correction of this separation document,
or other military records in the future, he is advised to pursue an
administrative remedy through his
USAR personnel managers prior to making application to the Board.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending his 25 June 2004 DD Form 214 by deleting the current entry in Item
12d and replacing it with the entry “12 06 08”; and by providing the
applicant a corrected separation documents that reflects this change.

The applicant requests correction of item 1 (Name [Last]), 12d (Total Prior Active Service), and item 12e (Total Prior Inactive Service) of his DD Form 214 (Certificate of Release or Discharge from Active Duty).
