THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) for the period ending 31 October 2001 be
corrected (1) in item 11 (Primary Specialty) to show he held military
occupational specialty (MOS) 88M for 21 years and 9 months; (2) in item 12b
(Separation Date this Period) to show he separated on 31 October 2003; (3)
in item 12c (Net Active Service this Period) to show he served 22 years, 0
months, and 11 days; (4) in item 19a (Mailing Address After Separation
(include Zip Code)) to show a mailing address of 4___ T___ O___Drive, Apt
1006, San Antonio TX 78271; and (5) in item 19b (Nearest Relative (Name
and Address – include Zip Code) to show the same address as in item 19a.
2. The applicant states that at the time he separated he only looked at
his DD Form 214 to ensure all his awards were entered.
3. The applicant provides the documents listed on his 20 April 2004 letter
attached to his application.
CONSIDERATION OF EVIDENCE:
1. After having had prior service in the Regular Army, the Army National
Guard, and the U. S. Army Reserve, he enlisted in the Regular Army on 20
October 1981. He completed advanced individual training and was awarded
MOS 64C (Motor Transport Operator) on or about 13 February 1981. Around
1987, MOS 64C was redesignated MOS 88M (Motor Transport Operator).
2. The applicant had continuous active duty until he was released from
active duty on 31 October 2003 for the purpose of retirement. His DD Form
214 for this period shows, in item 11, that he held MOS 88M for 19 years
and 9 months; in item 12b that he separated on 31 October 2001; in item 12c
that he completed 20 years and 11 days of net active service that period;
and in items 19a and 19b the mailing address 3___ W___ R___, San Antonio,
TX 78247. (His current mailing address is 2___ B___ V___, San Antonio, TX
78264.)
3. On 3 November 2003, the Taegu, Korea Transition Center requested the
U. S. Army Reserve Personnel Command to correct the applicant's DD Form
214 for the period ending 31 October 2001 as he currently requests.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record shows the applicant was released from active
duty on 31 October 2003 (not 2001) for the purpose of retirement. The
entries in items 11 and 12c of his DD Form 214 for that period were
calculated based on the erroneous separation date of 2001 in item 12b.
Correction of item 12b results in correction of item 11 to show he held MOS
88M for 21 years and 9 months and correction of item 12b to show he served
on active duty that period for 22 years and 11 days.
2. The dates in items 19a and 19b are used primarily for sending the
applicant correspondence immediately after his separation. (It is noted
that 5 months after his separation his address was neither 3___ W___ R___
San Antonio nor 4___ T___ O___Drive, Apt 1006 San Antonio.) At this point
in time, a correction to those items would be of no consequence. However,
since his transition point did request correction of those items within
days of his separation and since other corrections need to be made anyway,
the addresses in these items should be corrected, also.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending his DD Form 214 for the period ending 31 October 2001:
a. in item 11 to show he held MOS 88M for 21 years and 9 months;
b. in item 12b to show he separated that period on 31 October 2003;
c. in item 12c to show he completed 22 years and 11 days of net
active service that period; and
d. in items 19a and 19b to show an address of 4___ T___ O___Drive,
Apt 1006, San Antonio TX 78271.
