2. The applicant requests that his records be corrected to reflect award of the Purple Heart. In support of his request he submits a copy of the orders awarding him the Purple Heart, an extract from his service medical records indicating his treatment for shrapnel wounds, and a copy of his 1969 Department of Defense Form 214 (Armed Forces of the United States Report of Transfer or Discharge).
3. The applicant's military personnel file was not available to the Board. Information contained herein was reconstructed entirely from documents provided by the applicant.
4. The applicant served on active duty between 16 May 1967 and 1 March 1969. During his period of military service he was assigned in Vietnam for approximately 12 months. The exact dates of his assignment to Vietnam were not available to the Board. However, orders issued by the 41st Artillery Group on 22 May 1969, several months after the applicant's separation from active duty, awarded him the Purple Heart for wounds sustained on 19 January 1969. The extract from the applicant's service medical records, also submitted by him in support of his request, confirm that he was treated on 20 January 1969 for shrapnel wound to his right arm. His award of the Purple Heart was omitted from his separation document.
5. Department of the Army Pamphlet 672-3 notes that all individuals assigned to Vietnam were entitled to the Republic of Vietnam Gallantry Cross Unit Citation with Palm based on an award made to Headquarters, United States Army Vietnam and its subordinate units during the period 20 July 1965 to 28 March 1973.
CONCLUSIONS:
1. The evidence confirms that the applicant was awarded the Purple Heart for wounds sustained in Vietnam in January 1969. His records should be corrected accordingly.
2. The Board also notes that the applicant is entitled to the Republic of Vietnam Gallantry Cross Unit Citation with Palm. That award should also be added to his records.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected:
a. by showing that the individual concerned was awarded the Purple Heart; and
b. by showing that he is entitled to the Republic of Vietnam Gallantry Cross Unit Citation with Palm.

Item 40 (Wounds) on the applicant's DA Form 20 does not show he sustained wounds as a result of hostile action. There are no orders available to the Board which authorize award of the Purple Heart to the applicant. Based on the applicant’s dates of service in Vietnam, Appendix B of Army Regulation 600-8-22 shows that he participated in four campaigns of the Vietnam War.
ARMY | BCMR | CY2002 | 2002073346C070403
There are no orders in the applicant’s service personnel records which show that he was awarded the Purple Heart. The letter from the National Personnel Records Center stated essentially that a search of official military records failed to produce any evidence that the applicant received any wounds as a result of hostile action and that, in the absence of such evidence, a Purple Heart cannot be authorized. That all of the Department of the Army records related to this case be corrected by...

No other medical records were available to the Board, or provided by the applicant. The Board notes that the applicant arrived in Vietnam on 4 July 1969. Although the applicant may have received medical treatment while in Vietnam, there is no evidence which confirms the applicant was wounded as a result of hostile action.
ARMY | BCMR | CY2002 | 2002075363C070403
The applicant requests correction of his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) to show award of the Purple Heart and the Expert Marksmanship Qualification Badge with Rifle Bar. There are no orders in the applicant’s service personnel records which show that he was awarded the Purple Heart. However, there is no evidence of record, and the applicant has provided no medical evidence which shows he was wounded or treated for wounds as a result of...

There are no orders in the applicant’s service personnel records which show that he was awarded the Purple Heart. Department of the Army Pamphlet 672-3 (Unit Citation and Campaign Participation Credit Register) shows the applicant’s unit, 4th Battalion, 23rd Infantry, of the 25th Infantry Division, was also cited for award of the Republic of Vietnam Civil Actions Honor Medal First Class Unit Citation based on Department of the Army General Orders Number 51, dated 1971. However, evidence of...

The applicant states that he was never awarded the Purple Heart for a wound he received while in Vietnam. Subsequent to his separation, in 1996, the applicant submitted an application for correction of his military records requesting award of the Combat Infantryman Badge. However, notwithstanding that information, the Board notes that neither individual who submitted a statement supporting the applicant’s petition for award of the Purple Heart actually witnessed the applicant’s injury.
ARMY | BCMR | CY2002 | 2002081681C070215
There is no evidence in the applicant's service personnel records that he was awarded the Purple Heart or was wounded as a result of hostile action. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the Purple Heart is awarded for a wound sustained as a result of hostile action. However, evidence of record shows the applicant participated in two campaigns during his assignment in Vietnam which are not correctly shown by award of two bronze service stars for wear...
ARMY | BCMR | CY2002 | 2002077246C070215
The applicant's DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) does not show the Purple Heart as an authorized award. A review of the applicant's personnel records revealed that he is entitled to award of the Vietnam Service Medal with four bronze service stars, the Republic of Vietnam Gallantry Cross with Palm Unit Citation, the Republic of Vietnam Civil Actions Honor Medal First Class Unit Citation and the Meritorious Unit Commendation which are not shown...
ARMY | BCMR | CY2007 | 20070000961
Although, there are no general orders that show the applicant was awarded the Purple Heart and the applicant's name is not listed on the Vietnam Casualty Roster, the applicant's DA Form 20 clearly shows that he sustained shrapnel wounds to his left arm on 13 November 1968. Based on the foregoing, the evidence provided is sufficient to support the applicant's entitlement to award of the Purple Heart in this case. As a result, the Board recommends that all Department of the Army records of...

In accordance with Army Regulation 15-185, the application and the available military records pertinent to the corrective action requested were reviewed to determine whether to authorize a formal hearing, recommend that the records be corrected without a formal hearing, or to deny the application without a formal hearing if it is determined that insufficient relevant evidence has been presented to demonstrate the existence of probable material error or injustice. The applicant requests...
