2. The applicant requests that the award of the Purple Heart be added to his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge).
3. The applicant states his award of the Purple Heart is not reflected on his DD Form 214. In support of his request the applicant submits a copy of his award certificate for the Purple Heart
4. The applicant’s military records show that he was inducted into the Army of the United States on 11 April 1969, for a period of 2 years. He completed basic combat training at Fort Benning, Georgia, and advanced individual training at Fort Leonard Wood, Missouri.
5. The applicant served in the Republic of Vietnam from 10 September 1969 through 7 November 1970, with Company D, 26th Engineer Battalion.
6. On 8 November 1970, the applicant was honorably discharged under the provisions of Army Regulation 635-200, Overseas Returnee. His DD Form 214 indicates he had 1 year, 6 months and 28 days of creditable service, and was awarded the National Defense Service Medal, Vietnam Service Medal, Vietnam Campaign Medal with 60 Device, Mechanic Badge, Expert M-14 and Expert M-16 Badges.
7. By virtue of the applicant’s assignment with Company D, 26th Engineer Battalion, he is entitled to the Republic of Vietnam Gallantry Cross Unit Citation with Palm. He is also entitled to the Vietnam Service Medal with 4 Bronze Service Stars for his participation in following four campaigns, the Vietnam Summer-Fall 1969, the Vietnam Winter-Spring 1970, the DA Sanctuary Counteroffensive. and the Vietnam Counteroffensive Phase VII. The applicant’s DD Form 214 should be corrected to show these awards.
8. The Vietnam Casualty Report shows the applicant was wounded in action on
16 September 1970.
9. The applicant’s DA Form 20 (Enlisted Qualification Record) indicates he was wounded on 16 September 1970.
CONCLUSIONS:
1. Evidence shows the applicant was wounded in action on 16 September 1970, while serving in Republic of Vietnam, and is entitled to award of the Purple Heart.
2. The applicant is also entitled to the Republic of Vietnam Gallantry Cross Unit Citation with Palm, and the Vietnam Service Medal with 4 Bronze Service Stars.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by awarding the applicant the Purple Heart, the Republic of Vietnam Gallantry Cross Unit Citation with Palm, and the Vietnam Service Medal with 4 Bronze Service Stars.
