APPLICANT REQUESTS: That his DD Form 214 (Report of Separation from Active Duty), be corrected to show his correct social security number (SSN).
APPLICANT STATES: That his SSN was entered incorrectly at the time he entered into active service. It should be ___-__-4541, not ___-__-4241. He provides a copy of his social security card, his DD Form 214, and his Honorable Discharge Certificate as supporting evidence.
EVIDENCE OF RECORD: The applicant's military records show:
He enlisted in the Regular Army on 26 July 1974 under the SSN ___-__-4241. His DD Form 1584 (National Agency Check Request), dated 27 August 1974, was completed using the SSN ___-__-4241. All documents in his records show his SSN as ___-__-4241. His DD Form 214 shows that he was released from active duty on 16 July 1976 under SSN ___-__-4241. He was discharged from the U. S. Army Reserve on 25 July 1980 and his discharge certificate shows his SSN as ___-__-4241.
The applicant provides a copy of his social security card, which shows his SSN as ___-__-4541.
Army Regulation 635-5 prescribes the separation documents prepared for soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part it states that the DD Form 214 is a synopsis of the soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.
DISCUSSION: Considering all the evidence, allegations, and information presented by the applicant, together with the evidence of record, applicable law and regulations, it is concluded:
1. For historical purposes, the Army has an interest in maintaining the accuracy of its records. The data and information contained in those records should actually reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, this Board is reluctant to recommend that those records be changed.
2. Although the applicant’s SSN may have been ___-__-4541, he appropriately served on and was released from active duty under the SSN ___-__-4241 and discharged from the U. S. Army Reserve under the SSN ___-__-4241. While the Board understands the applicant’s desire to have the records changed, it finds no basis for compromising the integrity of the Army’s records. This Board action will be filed in his military records so a record of his SSN will be on hand.
3. In view of the foregoing, there is no basis for granting the applicant's request.
DETERMINATION: The applicant has failed to submit sufficient relevant evidence to demonstrate the existence of probable error or injustice.

The applicant states that his discharge is unjust and needs to be changed to honorable or upgraded to a better discharge. However, his DD Form 214 indicates that he was discharged with an undesirable discharge on 26 August 1974 under the provisions of Army Regulation 635-206, Section VI, for misconduct – conviction by a civil court or adjudged a juvenile. The Board noted the applicant's request for correction of item 9a (Type of Separation) on his DD Form 214 to show "honorable."
