         I certify that hereinafter is recorded the true and complete record of the proceedings of the Army Board for Correction of Military Records in the case of the above-named individual. A quorum was present during the further consideration and deliberation. The findings appearing in proceedings dated 28 March 2002 were affirmed. The following additional findings, conclusions, and recommendation were adopted by the Board.

         The Board convened at the call of the Director on the above date to reconsider the conclusions and recommendation appearing in proceedings dated 28 March 2002.

THE BOARD ADDITIONALLY FINDS:

19. Subsequent to the Board's initial action in this case (docket number AR2001062554), the applicant's DD Forms 214 issued on 3 October 1969 and on 18 November 1970 were located. The DD Form 214 issued on 3 October 1969 reflects that the applicant was awarded the ARCOM, VSM, VCM, and 3 OSB.

20. Based upon the additional information found in the two missing DD Forms 214, the Board determined the following:

         a. The period of service for award of the applicant's first GCMDL is 30 March 1966 through 29 March 1968, and the period of service for award of his second GCMDL is 30 March 1968 through 29 March 1971;

         b. The applicant is entitled to a third OSB in addition to the other awards already identified.

THE BOARD ADDITIONALLY CONCLUDES:

7. At the time of the Board’s 28 March 2002 decision in this case, it was the intent of the Board to make the applicant’s record as administratively correct as it should properly have been at the time. In this regard, it should be noted that the periods for award of the GCMDL are as stated in paragraph 20a. above, and that the applicant is entitled to a third OSB.

8. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records as recommended below.

THE BOARD RECOMMENDS:

1. That all of the Department of the Army records related to this case be corrected by amending the Board’s recommendation appearing in the proceedings, dated 28 March 2002, to show that the individual concerned was issued:

         a. A DD Form 215 correcting Item 2 on his DD Form 214 dated 29 March 1966 to show that his correct service number ends with the numerals "641;" and

         b. A DD Form 215 correcting his DD Form 214 dated 27 August 1973 to show that his correct service number (Item 2) ends with the numerals "641;" and that he is entitled to award of the ARCOM, AM, GCMDL with Oak Leaf Cluster, NDSM, VSM with 3 BSS, VCM, Republic of Vietnam Gallantry Cross with Palm Unit Citation, Republic of Vietnam Civil Actions Honor Medal, First Class Unit Citation, and three OSB.


2. That so much of the application as is in excess of the foregoing be denied.
