         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that the Driver and Mechanic Badge with Driver-W bar be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty).

3. The applicant makes no additional statement.

4. The applicant’s military records show that he enlisted in the Regular Army on 13 October 1993 after having had prior active service. He was awarded military occupational specialty 13M (Multiple Launch Rocket System Crewmember).

5. Headquarters, 6th Battalion, 37th Field Artillery, Permanent Orders 218-03, dated 5 August 1996, awarded the applicant the Driver and Mechanic Badge with Driver-W bar.

6. The applicant was honorably discharged on 2 March 1999. His DD Form 214 for the period 13 October 1993 through 2 March 1999 does not list the Driver and Mechanic Badge with Driver-W bar.

CONCLUSIONS:

1. There is sufficient evidence to show the applicant was awarded the Driver and Mechanic Badge with Driver-W bar and it should be added to his DD Form 214.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.

RECOMMENDATION:

That the applicant's DD Form 214 for the period13 October 1993 through 2 March 1999 be amended to add the Driver and Mechanic Badge with Driver-W bar.
