2. The applicant requests that his Certificate of Release or Discharge from Active Duty, DD Form 214, for the period ending 22 December 2000 item 12a be corrected to show he entered active duty in 1994 and item 12c to show he completed 6, not 10, years, of active service. He provides his DD Forms 214 for the periods ending 21 June 1991 and 22 December 2000 and his Army enlistment contract as supporting evidence.
3. The applicant’s military records show that he served on active duty in the U. S Navy from 22 June 1987 – 21 June 1991. He enlisted in the Regular Army on 2 March 1994 and separated on 22 December 2000. His DD Form 214 for the period ending 22 December 2000, item 12a shows he entered active duty on 2 March 1990. Item 12c shows he had 10 years, 9 months, and 21 days of active service this period.
CONCLUSIONS:
1. The applicant’s DD Form 214 for the period ending 22 December 2000 contains two errors in item 12. Item 12a should show that he entered active duty that period on 2 March 1994 (not 1990) and item 12c should show that he completed 6 years, 9 months, and 21 days of active service for that period (not 10 years, 9 months, and 21 days).
2. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That the applicant’s DD Form 214 for the period ending 22 December 2000 be corrected, in item 12a, to show he entered active duty on 2 March 1994 and, in item 12c, to show he completed 6 years, 9 months, and 21 days of active service for that period.
