2. The applicant requests that his DD Form 214 (Report of Transfer or Discharge) be corrected to show his correct date of entry and award of the Aircraft Crew Member Badge.
3. The applicant states that his DD Form 214 has the wrong date of entry. He also contends that he was awarded the Aircraft Crew Member Badge but it is not listed on his discharge document. In support of his application, he submits a copy of his DD Form 214.
4. The applicant’s military records show that he enlisted on 17 January 1966 for a period of 3 years. He served as a mechanic in Vietnam from 15 September 1966 through 14 September 1967 and was honorably released on 17 January 1969.
5. Item 17c (Date of Entry) on the applicant’s DD Form 214 shows the entry,
“17 Jan 69.” His DD Form 214 does not show the Aircraft Crew Member Badge as an authorized award.
6. The applicant’s DD Form 4 (Enlistment Record – Armed Forces of the United States), dated 17 January 1966, shows the applicant enlisted in the Regular Army on this date.
7. The applicant’s DA Form 20 (Enlisted Qualification Record) shows that he completed advanced individual training (AIT) in military occupational specialty (MOS) 67 (aircraft maintenance crewman) effective 27 May 1966.
8. The applicant’s DD Form 214 shows the Vietnam Service Medal as an authorized award. His records show that he served in two campaigns during his assignment in Vietnam.
9. Department of the Army Pamphlet 672-3 (Unit Citation and Campaign Participation Credit Register) lists the unit awards received by units serving in Vietnam. This document shows the unit, to which the applicant was assigned, was cited for award of the Meritorious Unit Commendation for actions during October 1965 to September 1966 by Department of the Army General Orders Number 17, dated 1968.
10. Department of the Army Pamphlet 672-3 (Unit Citation and Campaign Participation Credit Register) shows the applicant is entitled to award of the Republic of Vietnam Gallantry Cross with Palm Unit Citation based on Department of the Army General Orders Number 12, dated 1969. This award is not shown on his discharge document.
11. Department of the Army Pamphlet 672-3 (Unit Citation and Campaign Participation Credit Register) also shows the applicant is entitled to award of the Republic of Vietnam Civil Actions Honor Medal First Class Unit Citation based on Department of the Army General Orders Number 53, dated 1970. This award is not shown on his discharge document.
12. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that the Aircraft Crew Member Badge may be permanently awarded for successful completion of formal AIT in career management field 67 MOS’s.
CONCLUSIONS:
1. The applicant’s DD Form 4 (Enlistment Record) shows that he enlisted in the Regular Army on 17 January 1966 for a period of 3 years.
2. The applicant’s DD Form 214 incorrectly shows his date of entry as
17 January 1969. Therefore, it would appropriate to correct the applicant’s date of entry on his DD Form 214 at this time.
3. The applicant completed AIT in MOS 67A. Therefore, the applicant is entitled to correction of his DD Form 214 to show award of the Aircraft Crew Member Badge.
4. The applicant’s DD Form 214 shows the Vietnam Service Medal as an authorized award. However, evidence of record shows the applicant participated in two campaigns during his assignment in Vietnam which are not correctly shown by award of two bronze service stars for wear on his Vietnam Service Medal. Therefore, in accordance with the governing regulation, the applicant is entitled to the Vietnam Service Medal with two bronze service stars and correction of his DD Form 214 to show these awards.
5. The applicant’s unit received the Meritorious Unit Commendation, the Republic of Vietnam Gallantry Cross with Palm Unit Citation and the Republic of Vietnam Civil Actions Honor Medal First Class Unit Citation while he was assigned to it. Therefore, the applicant is entitled to correction of his DD Form 214 to show these unit awards.
6. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected:
a. by deleting the entry, “17 Jan 69” in item 17c (Date of Entry) on the DD Form 214 of the individual concerned;
b. by adding the entry, “17 Jan 66” in item 17c on the DD Form 214; and
c. by adding the entries, “Aircraft Crew Member Badge”, “Vietnam Service Medal with two bronze service stars”, “Meritorious Unit Commendation”, “Republic of Vietnam Gallantry Cross with Palm Unit Citation” and “Republic of Vietnam Civil Actions Honor Medal First Class Unit Citation” in item
24 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) on his DD Form 214.
