2. The applicant requests reconsideration of his request that his Report of Transfer or Discharge, DD Form 214, be corrected to show he was awarded the Bronze Star Medal, two awards of the Army Commendation Medal, the Marksman Qualification Badge with Machine Gun Bar, a Certificate of Outstanding Accomplishment, and a Certificate of Appreciation. He provides as supporting evidence a 15 August 1969 letter stating he was to receive a Certificate of Appreciation, a photograph showing he was awarded that Certificate of Appreciation, a Certificate of Outstanding Accomplishment issued to the Officers and Men of the 472d Medical Detachment, a Certificate of Appreciation in recognition of his active service, a Certificate of Training showing he completed the 10-week Fort Jackson Leader School, and a 23 July 1969 letter showing he was recommended for award of the Bronze Star Medal.
3. Incorporated herein by reference are military records which were summarized in Proceedings prepared to reflect the Board's original consideration of his case on 1 May 2001 (docket number AR2001051381).
4. The provided Certificate of Training showing the applicant completed the 10-week Fort Jackson Leader School is verified by information contained on his Enlisted Qualification Record, DA Form 20, item 27 (Military Education).
5. Army Regulation 635-5, in effect at the time, provided that item 25 of the DD Form 214 would reflect installation training courses, military correspondence courses, and off-duty courses the enlisted person had completed successfully during the period covered by the DD Form 214. The applicant’s DD Form 214, item 25 lists “None” for education and training completed.
CONCLUSIONS:
1. As the applicant was informed in the original consideration of his case, Certificates of Achievement, Letters of Appreciation, and similar documents are not recorded on the DD Form 214. However, the certificates he provides with this application will be filed in his Official Military Personnel File with this Board action.
2. The applicant provides no new evidence that was not available during the original consideration of his case concerning the Bronze Star Medal, the two awards of the Army Commendation Medal, and the Marksman Qualification Badge with Machine Gun Bar.
3. Item 25 of the applicant’s DD Form 214 should be amended to show he completed the 10-week Fort Jackson Leader School.
4. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
1. That the applicant’s DD Form 214, item 25, be amended to show he completed the 10-week Fort Jackson Leader School in 1962.
2. That so much of the application as is in excess of the foregoing be denied.
