2. The applicant requests correction of item 14 (Military Education) on her DD Form 214 (Certificate of Release or Discharge from Active Duty).
3. The applicant states that she wants to get credit in college for her military education. In support of her application, she submits a course completion certificate, dated 26 October 1999, for the Medical Specialist Sustainment Military Occupational Specialty (MOS) 91B10 correspondence course; a completion certificate for the Driver’s Training Academy HMMWV Operator’s Course; a Memorandum for Record pertaining to completion of a Suture Certification Course, dated 18 September 1998; a diploma from the Eastern College of Health Vocations for completion of the course of study in Dental Assistant, dated 28 October 1990; an academic transcript from the Eastern College of Health Vocations, dated 17 May 1996; a Memorandum, dated
1 October 1998, which states the applicant completed a 40-hour Driver’s Course for a Transportation Motor Pool (TMP) license; an application for admission/credit registration to Central Texas College, dated 27 March 1998; a Central Texas College Grade Report, dated 2 June 1998; a Permanent Academic Record from the Central Texas College, dated 23 October 1998; and a certificate of affiliation with the United States Army Medical Department Regiment.
4. The applicant’s military records show that she enlisted on 19 November 1996 for a period of 4 years. She served as a medical specialist and was honorably discharged on 16 January 1999 by reason of physical disability with severance pay.
5. Item 14 (Military Education) on the applicant’s DD Form 214 shows she completed the German Language Orientation Course in June 1997, the Medical Specialist Course in April 1997 and the Emergency Medical Technician Course in June 1998.
6. The applicant provided a course completion certificate, dated 26 October 1999, which shows she completed all requirements for the Medical Specialist Sustainment MOS 91B10 correspondence course on 25 October 1999.
7. The applicant provided a completion certificate for the Driver’s Training Academy HMMWV Operator’s Course which shows she completed the 40-hour course in June 1997.
8. The applicant also provided a Memorandum for Record, dated 18 September 1998, which shows she successfully completed the Suture Certification Course which included a one hour lecture and two hour hands on evaluation.
9. The applicant provided a diploma which indicates that she completed a course of study in Dental Assistant on 28 October 1990. She also provided an academic transcript for courses she completed in 1990 and documentation pertaining to college courses completed at Central Texas College.
10. The applicant provided a Memorandum, dated 1 October 1998, which shows she completed a 40-hour class for the Driver’s Course for the TMP.
11. The applicant also provided a certificate of affiliation with the United States Army Medical Department Regiment.
12. Army Regulation 635-5 establishes the policies and procedures for completion and distribution of the DD Form 214. In pertinent part it states that item 14 (Military Education) will list formal in service training courses
of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed. This information is to assist the soldier after separation in job placement and counseling; therefore, training courses for combat skills are not listed.
CONCLUSIONS:
1. Evidence of record shows the applicant completed the Medical Specialist Sustainment MOS 91B10 correspondence course on 25 October 1999. However, in accordance with the governing regulation, correspondence courses are not shown on the DD Form 214.
2. The applicant completed the 40-hour Driver’s Training Academy HMMWV Operator’s Course in June 1997. Therefore, the applicant is entitled to correction of item 14 (Military Education) on her DD Form 214 to show this course.
3. Evidence of record shows the applicant completed the Suture Certification Course which included a one hour lecture and two hour hands on evaluation. However, in accordance with the governing regulation, formal in service training courses of 40 hours or more will be shown in item 14 on the DD Form 214. Therefore, there is no basis for showing this course on the applicant’s DD
Form 214.
4. The Board considered the diploma provided by the applicant which indicates she completed a course of study in Dental Assistant on 28 October 1990, the academic transcript for courses the applicant completed in 1990 and the documentation pertaining to college courses she completed at Central Texas College. However, in accordance with the governing regulation, item 14 will list formal in service training courses of 40 hours or more successfully completed during the period of service covered. Therefore, there is no basis for showing this documentation in item 14 on the applicant’s DD Form 214.
5. The applicant completed a 40-hour class for the Driver’s Course for the TMP. Therefore, she is entitled to correction of item 14 on her DD Form 214 to show this course.
6. The Board considered the applicant’s contention that her affiliation with the United States Army Medical Department Regiment should be shown in item
14 on her DD Form 214. However, the United States Army Medical Department Regiment is not a formal in service training course of 40 hours or more. Therefore, there is no basis for showing this affiliation in item 14 on her DD
Form 214.
7. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records, but only as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected by showing the entries, “Driver’s Training Academy HMMWV Operator’s Course, 1 week, June 1997” and “Driver’s Course for the Transport Motor Pool, 1 week, June 1998” in item 14 (Military Education) on the DD
Form 214 of the individual concerned.
2. That so much of the application as is in excess of the foregoing be denied.
