2. The applicant requests that his records be corrected to reflect award of a Meritorious Service Medal, three awards of the Army Commendation Medal, and completion of a Faculty Development Course. He states the information was omitted from his final DD Form 214 (Certificate of Release or Discharge from Active Duty.)
3. Information available to the Board indicates the applicant entered active duty in October 1979 and served continuously until retirement in October 2001. Documents provided by the applicant indicate he was awarded three Army Commendation Medals (9205, 9512, 9807) and one Meritorious Service Medal (0106). However, only two Army Commendation Medals are recorded on his October 2001 separation report. His third Army Commendation Medal and the Meritorious Service Medal were omitted.
4. In September 1988 the applicant successfully completed a two week Faculty Development Course (course number 5K-F3/520-F3) at Fort Sam Houston, Texas. That course was omitted from item 14 (military education) of his 2001 separation document although other courses, completed in 1980, 1981, 1984, 1988, and later, are recorded.
5. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. In pertinent part, it states that item 13 (decorations, medals, badges, citations and campaign ribbons), for all periods of service, will be recorded. It also states, in effect, that item 14 (military education) will list formal in service training courses successfully completed during the period of service covered by title, length in weeks, and month and year completed. This information is to assist the soldier after separation in job placement and counseling; therefore, training courses for combat skills are not listed. Although this item should reflect the month and year of completion, the applicant's separation report reflects only the year he completed the courses that are recorded.
CONCLUSIONS:
1. Evidence available to the Board confirms the applicant was awarded three awards of the Army Commendation Medal and one Meritorious Service Medal. His records should be corrected accordingly.
2. The evidence also confirms that the applicant completed a 2 week Faculty Development Course at Fort Sam Houston, Texas in September 1988. However, in view of the fact that his separation report records only the year of completion for his other courses, the Board, as an exception to policy, concludes it would be appropriate to correct item 14 of his DD Form 214 to show that he completed the Faculty Development Course in 1988, and omit the month of completion. The entry will then be consistent with the other entries in that item and serves the intended purpose.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected:
a. by showing that the individual concerned was awarded three Army Commendation Medals and a Meritorious Service Medal; and
b. by showing in item 14 (military education) that the applicant completed the 2 week, Faculty Development Course in 1988.
