APPLICANT REQUESTS: In effect, that item 4 of his Report of Medical Examination, SF 88, be changed to show his address as ___ Midwest City, OK.
He states that the error was a typographical error. He provides the SF 88 and a letter from his wife as supporting evidence.
EVIDENCE OF RECORD: The applicant's military records show:
He completed his enlistment physical examination on 6 June 1996. Item 4 of the SF 88 shows his home address as ___ Norman, OK. Item 14 shows his next of kin as his mother at the same address. He completed a Questionnaire for National Security Positions, SF 86, on 25 July 1997. Item 9 shows he lived at ___ Norman, OK from February 1991 – April 1997. Item 44 shows his mother lived at ___ Norman, OK. Item 44 shows that he married on 22 July 1997. He
enlisted in the Regular Army on 27 August 1997.
DISCUSSION: Considering all the evidence, allegations, and information presented by the applicant, together with the evidence of record, applicable law and regulations, it is concluded:
1. In order to justify correction of a military record the applicant must show to the satisfaction of the Board, or it must otherwise satisfactorily appear, that the record is in error or unjust. The applicant has failed to submit evidence that would satisfy this requirement.
2. The evidence of record shows that on 6 June 1996, when the SF 88 was completed, the applicant and his mother live at ___ Norman, OK.
3. In view of the foregoing, there is no basis for granting the applicant's request.
DETERMINATION: The applicant has failed to submit sufficient relevant evidence to demonstrate the existence of probable error or injustice.
