2. The applicant requests that the award of the Army Commendation Medal be shown on his DD Form 214. He states that his DD Form 214 (Certificate of Release or Discharge from Active Duty) does not show that he was awarded this medal.
3. The applicant entered the Army on 27 September 1988, completed training, and served in Southwest Asia during Operation Desert Storm from October 1990 to April 1991. He was awarded the Good Conduct Medal, among other awards, and on 3 July 1991 he was awarded the Army Commendation Medal for meritorious service during the period 17 January 1991 through 1 March 1991, per Headquarters, 1st Brigade, 2nd Armored Division Permanent Order 08-008.
4. The applicant was released from active duty for unsatisfactory performance with an honorable character of service on 20 November 1991. His DD Form 214 does not reflect award of the Army Commendation Medal.
CONCLUSIONS:
1. The applicant was awarded the Army Commendation Medal and this award should be depicted on his DD Form 214.
2. In view of the foregoing findings and conclusion, it would be appropriate to correct the applicantÂ’s record as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by showing that the individual
concerned was awarded the Army Commendation Medal and that this award be depicted on his DD Form 214.
