2. The applicant requests that his DD Form 214, Certificate of Release or Discharge from Active Duty, item 23, Type of Separation, have Honorable entered.
3. He states that item 23 on his DD Form 214 was inadvertently left blank.
4. His military records show that he was retired for years of service on 28 February 1994. The DD Form 214 he was issued has both item 23 and item 24, Character of Service, left blank.
5. Army Regulation 635-5 provides guidance on the entries to be made on DD Forms 214. This regulation specifies that item 23 must have one of seven types of separation entered, one of which being Retirement, and item 24 must have one of six characterizations of service entered, one of which being Honorable.
CONCLUSIONS:
1. The applicants DD Form 214, items 23 and 24, have both been left blank in contravention with Army Regulation 635-5.
2. The applicant was retired for years of service, denoting long, honorable service.
3. In view of the foregoing, the applicants records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected:
a. by showing that DD Form 214 of the individual concerned for the period ending 28 February 1994 have Retirement entered in item 23, Type of Separation; and
b. by entering Honorable in his DD Form 214, item 24, Character of Service.
