2.  The applicant requests correction of his military records to show his date of birth as 29 August 1935.

3.  The applicant states, in effect, that he was not able to obtain the certificate of his birth at the early time of his service until recently when Israel permitted his relatives to send it to him.  He knew all along of the error.

4.  A copy of a Certificate of Registration of Birth, Department of Health, Government of Palestine, indicates that the applicant was born on 29 August 1935.

5.  Two Department of Defense (DD) Forms 214 (Armed Forces of the United States Report of Transfer or Discharge), which the applicant signed, indicate that the applicant’s date of birth was 29 August 1935, and that he was honorably discharged from the United States Air Force on 22 June 1963 and 16 May 1967.

6.  A statement by Purdue University indicates that the applicant’s date of birth was 29 August 1935, and that he was granted a Master of Arts degree on 8 June 1969.

7.  A DD Form 4 (Enlistment Contract-Armed Forces of the United States) indicates that the applicant’s date of birth was 8 August 1939, and that he enlisted in the Kansas Army National Guard-Reserve Air Force on 14 September 1974.

8.  A National Guard Bureau (NGB) Form 22 (Report of Separation and Record of Service), which the applicant signed, indicates that his date of birth was 8 August 1939, and that he was honorably discharged from the Air National Guard of Kansas and as a Reserve of the Air Force on 13 September 1976.

9.  A DD Form 4, which the applicant signed, indicates that
the applicant’s date of birth was 8 August 1939, and that he enlisted in the Air National Guard of the United States on 14 September 1976.



10.  A NGB Form 22 indicates that the applicant’s date of birth was 8 August 1939, and that he was honorably discharged from the Air National Guard of Kansas and as a Reserve of the Air Force on 22 September 1983.

11.  A DD Form 1966/1 (Application for Enlistment-Armed Forces of the United States), which the applicant signed, indicates that his date of birth was 8 August 1939, and that he enlisted in the Army National Guard of Kansas and as a Reserve of the United States Army on 23 September 1983.

12.  A DD Form 4/1 (Enlistment/Reenlistment Document-Armed Forces of the United States) indicates that the applicant’s date of birth was 29 August 1935, and that he enlisted in the Army National Guard of the United States on 23 September 1983.

13.  A Department of the Army (DA) Form 873 (Certificate of Clearance and/or Security Determination), dated 24 April 1984, indicates that the applicant’s date of birth was 8 August 1939.

14.  A DD Form 214 (Certificate of Release or Discharge from Active Duty), which the applicant signed, indicates that his date of birth was 8 August 1939, and that he served on active duty from 30 September 1983 through 31 January 1988 while a member of the Army National Guard.

15.  A NGB Form 22, as amended, indicates that the applicant’s date of birth was 8 August 1939, and that he was honorably separated from the Army National Guard of Kansas on 1 February 1988.

16.  A DD Form 398 (Department of Defense Personnel Security Questionnaire), which the applicant signed on 1 August 1988, indicates that his date of birth was 8 August 1939.

17.  A DD Form 4/1, signed by the applicant indicates that his date of birth was 8 August 1939, and that he enlisted in the United States Army Reserve on 21 December 1992.


18.  A DD Form 1172 (Application for Uniformed Services Identification Card DEERS Enrollment) indicates that the applicant’s date of birth was 8 August 1939, and that his spouse received her card on 4 December 1993.

19.  A DA Form 5016-R (Chronological Record of Military Service), dated 15 December 1988, a DA Reserve Personnel (DARP) Form 249 (Chronological Statement of Retirement Points), dated 23 October 1992, and a DARP Form 249-2-E (Chronological Statement of Retirement Points), dated 14 September 1993, indicate that the applicant’s date of birth was 8 August 1939.  A DARP Form 249-2-E, dated 15 November 1994, indicates that his date of birth was 29 August 1935.

20.  A DA Form 2A (Personnel Qualification Record (Enlisted)), dated 26 March 1993, initially indicated that the applicant’s date of birth was 8 August 1939.  However, it was lined through and his date of birth was shown as 29 August 1935.

21.  On 17 July 1995, the applicant was assigned to the Retired Reserve, United States Army Reserve Personnel Center.

CONCLUSIONS:

1.  The available military records indicate that the applicant’s date of birth was shown as 29 August 1935 while he was a member of the United States Air Force, at the time of his enlistment in the Army National Guard of the United States on 23 September 1983, and on a DARP Form 249-2-E, dated 15 November 1994.

2.  The Board notes that the applicant’s date of birth was shown as 8 August 1939 during his remaining military service, and that he verified it as that date on several occasions.





3.  Notwithstanding the foregoing and in view of the Certificate of Registration of Birth, the Board concludes that it would be appropriate to correct the applicant’s military records to show his date of birth as 29 August 1935.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by showing the date of birth of the individual concerned as 29 August 1935.
