2.  The applicant requests correction of his military records to reflect award of the Purple Heart.  He notes the award was omitted from his separation report.

3.  Records available to the Board indicate the applicant entered active duty on 17 October 1967 and served in Vietnam as an infantryman between September 1968 and August 1969.  On 29 November 1968 he sustained a fragment wound as a result of hostile action and was awarded the Purple Heart.  Orders confirming award of the Purple Heart were published by the 9th Infantry Division on 21 February 1969.

4.  Additionally, while the applicant was assigned to Vietnam he was awarded an Air Medal for meritorious service during the period 2 December 1968 through 30 March 1969, an Army Commendation Medal (680927-690221), and two Army Commendation Medals with “V” devices (19 October 1968 and 
23 April 1969).  Each of the awards were confirmed in orders published by the 9th Infantry Division in Vietnam.

5.  On 29 August 1969 he was released from active duty to attend school.  While his DD Form 214 reflect award of one Army Commendation Medal it does not reflect award of the Purple Heart, Air Medal or either Army Commendation Medal for valor.

CONCLUSIONS:

1.  The evidence confirms the applicant was awarded the Purple Heart, an Air Medal, an Army Commendation Medal for meritorious achievement and two Army Commendation Medals with “V” devices.

2.  In view of the foregoing, it would be appropriate to correct the applicant’s records as recommended below.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected:

	a.  by showing that the individual concerned was awarded the Purple Heart for wounds sustained as a result of hostile actions on 29 November 1968;

	b.  by showing that he was awarded an Air Medal and Army Commendation Medal for meritorious achievement; and

	c.  by showing that he was awarded two Army Commendation Medals with “V” device.
