2. In effect, the applicant requests that his DD Form 214 (Report of Transfer or Discharge) be corrected to show his date of birth as 4 June 1941 vice 4 July 1941, and that it show he is a high school graduate.
3. The applicant entered the Army on 17 November 1966 and was released from active duty on 15 November 1968. His
DD Form 214 shows his date of birth as 4 July 1941 and the number of years of education completed as 8 years of elementary school.
4. Various official documents, including his DD Form 47 (Record of Induction), his Report of Medical Examination, his DA Form 20 (Enlisted Qualification Record), a DA Form 3027 (Request for National Agency Check), and his DD Form 398, show his date of birth as 4 June 1941.
5. Item 9 (Education) of the applicants DD Form 398, which he signed, shows that he attended a public school in Missouri from 1946 to 1954, graduated, and again from 1954 to 1959 and did not graduate. His DA Form 20 indicates that he had 8 years of education, completing his education in 1955.
CONCLUSIONS:
1. The applicants date of birth is 4 June 1941. His
DD Form 214 should be changed accordingly.
2. The applicant may have had more than 8 years of education; however, the Board cannot determine his educational qualifications based on the records available.
3. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicants records as recommended below.
RECOMMENDATION:
1. That all of the Department of the Army records related to this case be corrected by showing that the individual concerned was born on 4 June 1941.
2. That so much of the application as in excess of the foregoing be denied.
