1. Attached is DD Form 149, Application for Correction of Military Records, dated 8 March 1994, submitted by the subject individual who seeks correction of his records to show his Prisoner of War (POW) status and to be issued a Purple Heart. His records were lost or destroyed in the fire that occurred at the Records Center in 1973.
2. Since your headquarters has already determined that he is entitled to the POW Medal as indicated by the enclosed DA Form 1577, it is requested that DD Form 215 be issued correcting his WD AGO Form 53-55 to show he was in a POW status between 19 December 1944 and 29 April 1945. Also, since the WD AGO Form 53-55 shows that he is entitled to the Purple Heart, he desires that the medal be issued to him.
