He states that he received the Purple Heart for the one wound that he received on 5 March, and for the wounds that he received on 30 April and 4 May, but not for the other two wounds. However, a clinical record cover sheet indicates that the applicant was wounded in the left leg and the left shoulder, that he was admitted to the 36 th on 7 March 1967 and returned to duty on 12 March 1967. The 24 October 1967 report of medical history shows that the applicant indicated that he received a...

He states, in effect, that he was wounded in Vietnam in November 1971, but the Purple Heart was not reflected on his DD Form 214. Army Regulation 600-8-22 provides, in pertinent part, that the Purple Heart is awarded for a wound sustained as a result of hostile action. While the medical records do not make clear that his wounds were the result of hostile action, collateral documentation shows that he was, at the time of his evacuation, a squad leader with an infantry unit involved in...

In this case, the applicant's name is not shown on the Vietnam casualty roster, his DA Form 20 is not available for review, his service record is void of any orders that show he was awarded the Purple Heart, his record is void of any official Army telegrams or Western Union telegrams, and there is no conclusive evidence...

Orders show the applicant received the Purple Heart for wounds received in action on 23 October 1969 in Vietnam. Since there is no evidence of record which shows that he was wounded as a result of hostile action on any date other than 23 October 1969, there is insufficient evidence on which to base a second award of the Purple Heart in this case. The applicant participated in three campaigns during his assignment in Vietnam which entitles him to award of the Vietnam Service Medal with...
