APPLICANT REQUESTS THAT:

His Fitness Assessment (FA) failures, dated 10 Jul 12 and 16 Oct 
12, be removed from the Air Force Fitness Management System 
(AFFMS).

APPLICANT CONTENDS THAT:

He was diagnosed with Post-Traumatic Stress Disorder (PTSD), 
depression, anxiety, and an adjustment disorder.  Changes to 
medications, moods, and mental states prevented him from 
achieving a passing score on his FAs.  

His AF Forms 108, Physical Fitness Education and Intervention 
Processing, completed by his medical provider reflect that he 
had a medical condition that precluded him from achieving a 
passing fitness score.

In support of his request, the applicant provides copies of his 
AF Forms 108.

STATEMENT OF FACTS:

The applicant is currently serving in the Regular Air Force in 
the grade of technical sergeant.  

The applicant’s last five FA scores are as follows:

AIR FORCE EVALUATION:

AFPC/DPSIM recommends denial.  DPSIM states without validation 
from his commander to remove the FAs, they must recommend 
denying the relief sought.

AFI 36-2905, Fitness Program, Air Force Guidance Memorandum 
(AFGM) 3, paragraph 10a, states that “If the medical evaluation 
validates the illness/injury, the Unit Commander may invalidate 
the test results.”

DPSID further states that the applicant provided his AF Forms 
108; however, there was not any documentation from the unit 
commander to invalidate the test results.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

In response to the Air Force evaluation the applicant provided 
letters from his medical care providers, and his commander 
supporting the removal of his FAs dated 10 Jul 12 and 16 Oct 12.  

The applicant's complete response is at Exhibit D.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
corrective action.  We note the office of primary responsibility 
recommends denial based on the fact the applicant did not 
provide validation from his commander to remove the FAs.  
However, the applicant has submitted letters from his medical 
care providers and his commander requesting that the 10 Jul 
12 and 16 Oct 12 FAs be removed from the AFFMS.  Therefore, in 
view of the above, we find the evidence presented sufficient to 
grant the requested relief and recommend the records be 
corrected as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air 
Force relating to APPLICANT, be corrected to show that his 
Fitness Assessments (FAs) dated 10 Jul 12 and 16 Oct 12, be 
declared void and removed from the Air Force Fitness Management 
System (AFFMS).
