APPLICANT REQUESTS THAT:
Her Fitness Assessment (FA) dated 9 Apr 12, be removed from the
Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT:
She injured her shoulder (rotator cuff) during her FA and was
subsequently seen at the Medical Treatment Facility (MTF) for her
injury. Her FA was erroneously updated because she was unable to
complete her test due to the injury.
In support of her request, the applicant provides a copy of VA
Form 10-5345, Request for and Authorization to Release Medical
Records or Health Information.

Exhibit A.

STATEMENT OF FACTS:
The applicant is currently serving in the Regular Air Force in
the grade of staff sergeant.
On 24 Oct 12, AFPC/DPSIM requested the applicant provide
additional supporting documentation to substantiate her claim.
Specifically, copies of AF Form 108, Physical Fitness Education
and Intervention Processing, signed score sheet and fitness
questionnaire.

AIR FORCE EVALUATION:
AFPC/DPSIM recommends denial. DPSIM states that the applicant
has not provided any medical documentation or a commanders
memorandum to validate the MTFs assessment of her injury.
DPSIM states they requested additional documents but none were
received.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 25 Apr 13, for review and comment within 30 days
(Exhibit D). As of this date, this office has not received a
response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. After
thoroughly reviewing the evidence of record and noting the
applicants contentions, we are not persuaded the contested FA
is in error or unjust. We note the applicant has failed to
provide the requested documentation requested by AFPC/DPSIM to
support the requested relief. Specifically, a signed score
sheet and fitness questionnaire and a copy of her AF Form 108.
Based on the evidence before us, we do not believe the applicant
has met the burden of proving the existence of an error or an
injustice in her record. Therefore, we find insufficient
evidence to warrant disturbing the record. However, should the
applicant provide such evidence, we would be willing to
reconsider her request. In view of the above and in the absence
of evidence to the contrary, we find no basis to recommend
granting the relief sought in this application.

The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that
the application was denied without a personal appearance; and
that the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered
