APPLICANT REQUESTS THAT:
His DD Form 214, Certificate of Release or Discharge from Active
Duty, be corrected to reflect that he was awarded the Air Force
Achievement Medal (AFAM).

APPLICANT CONTENDS THAT:
He requested copies of his military awards and discovered the
AFAM was not listed amongst his entitlements. He presumed it is
not reflected on his DD Form 214 because it was awarded in close
proximity to his discharge.
He is in poor health and would like to put his affairs in order
as soon as possible.

STATEMENT OF FACTS:
The applicants military personnel records indicate he enlisted
in the Regular Air Force on 15 Dec 82 and served on active duty
until 26 Sep 86 when he was released from active duty for
strength reduction with service characterized as honorable.

AIR FORCE EVALUATION:
AFPC/DPSID recommends denial, indicating that a review of the
applicants military personnel records failed to reveal any
documentation indicating the applicant had been awarded the
AFAM. Additionally, all administrative avenues have not been
exhausted for a retroactive request of an AFAM, as well as, the
request for consideration of an AFAM is incomplete. For a
complete package, the following information should be included:
a signed recommendation from someone with firsthand knowledge of
the act or achievement, eyewitness statements attesting to the
act(s) of outstanding meritorious achievement or service
performed, sworn affidavits, certificates, and any other related
documentation; referral by a Member of Congress; and a proposed
citation.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 23 Oct 12 for review and comment within 30 days.
As of this date, no response has been received by this office
(Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations. We note the OPRs comments
concerning the requirements of Title 10, United States Code,
Section 1130, enacted as part of the Fiscal Year 1996 National
Defense Authorization Act; however, the applicant is not
requesting that he be awarded the AFAM, but rather that a
previously awarded AFAM be added to his DD Form 214.
2. The application was not timely filed; however it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. After
thoroughly reviewing the evidence of record and noting the
applicants contention, we are not persuaded the AFAM should be
added to his DD Form 214. In this respect, we note that
although the applicant contends the AFAM was not reflected on
his DD Form 214 because it was awarded in close proximity to his
discharge, he has failed to provide, nor do his records contain,
any corroborative evidence, i.e., copies of the orders awarding
the AFAM and/or the AFAM certificate/citation, to support this
contention. Therefore, in the absence of evidence he was
previously awarded the AFAM, we find no basis upon which to
recommend granting the requested relief.

THE BOARD DETERMINES THAT:
The applicant be notified the evidence presented did not
demonstrate the existence of material error or injustice; the
application was denied without a personal appearance; and the
application will only be reconsidered upon the submission of
newly discovered relevant evidence not considered with this
application.
