APPLICANT REQUESTS THAT: 

 

His 26 Apr 12 Fitness Assessment (FA) score be removed from the 
Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT: 

 

He had surgery on 15 Dec 10, which resulted in him having a 
hernia mesh implant. On 26 Apr 12, he failed his FA due to side 
effects from his surgery. 

 

The applicant’s complete submission, with attachment, is at 
Exhibit A.

STATEMENT OF FACTS: 

 

The applicant is currently serving in the Regular Air Force in 
the grade of staff sergeant (E-5). 

 

On 26 Apr 12, the applicant participated in the contested FA, 
attaining a composite score of 61.70, which constituted an 
unsatisfactory assessment. 

 

IAW Air Force Instruction 36-290, Fitness Program, Paragraph 
2.3.2., all members must complete a Fitness Assessment 
Questionnaire (FSQ) prior to taking their FA. The FSQ should be 
completed no earlier than 30 calendar days (90 days for Air 
Reserve Component (ARC)), but NLT 7 days prior to FA to provide 
time for medical evaluation, when indicated. (Note: Failure to 
complete FSQ does not invalidate the FA.) A medical provider 
must evaluate all members with health issues identified on the 
FSQ prior to the FA. The member must notify the Unit Fitness 
Program Manager of the assessment/training clearance status from 
the provider. 

 

The remaining relevant facts pertaining to this application are 
contained in the letter prepared by the appropriate office of 
the Air Force, which is attached at Exhibit C.

AIR FORCE EVALUATION: 

 

AFPC/DPSIM recommends denial, noting the applicant did not 
provide any documentation to substantiate a pre-existing 
condition which would exempt his participation in the 26 Apr 12 
fitness assessment. 

 

A complete copy of the AFPC/DPSIM evaluation, with attachments, 
is at Exhibit C.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A complete copy of the Air Force evaluation was forwarded to the 
applicant on 15 Oct 12 for review and comment within 30 days. 
As of this date, no response has been received by this office 
(Exhibit D).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an injustice. The applicant 
contends his hernia mesh implant caused his 26 April 2012 
Fitness Assessment (FA) failure. We note the comments of the 
Air Force Office of primary responsibility indicating the 
applicant did not provide any medical documentation to 
substantiate his claim that he should have been exempt from the 
contested FA; however, after a careful review of the applicant’s 
complete submission, we believe a preponderance of the evidence 
indicates that relief is warranted. In this respect, we note 
the applicant’s commander provided a supporting statement citing 
a letter provided by the applicant’s doctor and noting his 
consistent performance on previous FA’s as the basis for his 
belief the applicant’s FA failure was indeed attributable to his 
mesh implant. Additionally, we note the applicant was exempt 
from the cardio, sit-up, and push-up components of his FA that 
followed. Therefore, in view of the above, we recommend the 
applicant’s records be corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to the APPLICANT be corrected to show that his 

 


26 April 2012 Fitness Assessment be declared void and removed 
from the Air Force Fitness Management System (AFFMS).
