APPLICANT REQUESTS THAT:
His official military records be corrected by having his Fitness
Assessment (FA), dated 27 Oct 11, declared void and removed.

APPLICANT CONTENDS THAT:
He had knee surgery to repair a torn anterior cruciate ligament
(ACL) and meniscus in Apr 11. During his 27 Oct 11 FA, his knee
became very painful so he was unable to continue running on it.
He went to his health provider that afternoon, and the health
provider realized he should not have been cleared for the cardio
portion of the FA and generated a new AF Form 422, Notification
of Air Force Members Qualification Status. He was able to
retest a week later on the components of the FA he was cleared
to take, and passed without any problems.
In support of his request, the applicant provides copies of a
memo of support from his commander, two fitness score sheets, an
AF Form 422, and a Duty Limiting Condition Report.

STATEMENT OF FACTS:
The relevant facts pertaining to this application are contained
in the letter prepared by the appropriate office of the Air
Force, which is attached at Exhibit C.

AIR FORCE EVALUATION:
HQ AFPC/DPSIM recommends annotating the cardio portion of the
applicants 27 Oct 11 FA with exempt.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 22 Dec 11 for review and comment within 30 days.
As of this date, no response has been received by this office
(Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. The member
is requesting that his Fitness Assessment (FA), dated 27 Oct 11,
be declared void and removed from Air Force Fitness Management
System. While we note the recommendation from HQ AFPC/DPSIM
that the record be corrected to reflect the member was exempt
from the cardio portion of the FA, we believe given the
potential impact of his knee injury on the other components of
the test, it is proper and fitting to recommend the entire
fitness assessment be invalidated as the applicant has
requested. Therefore, we recommend the applicants records be
corrected as indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to the APPLICANT be corrected to show that his
Fitness Assessment (FA), dated 27 Oct 11, be declared void and
removed from the Air Force Fitness Management System (AFFMS).

On 29 Nov 2011, a medical evaluation letter was signed by the same provider who issued the previous AF Form 469s.
